门诊药房退药处方分析与探讨

Analysis and Discussion on Drug Repercussion Prescription in Outpatient Pharmacy

目的：了解门诊药房退药的主要原因，减少不合理退药发生，保证患者用药安全。方法统计某院门诊药房干预前2014年1月~11月所有退药处方，按退药原因、退药种类等进行分析，然后采取相应的措施进行干预；并与干预后2015年1月～ 11月所有退药处方进行对比分析。结果与干预前相比，退药处方数由870张减少到530张，退药原因发生一定变化，退药种类基本无变化。结论通过门诊退药处方的分析，采取相应的干预措施，可有效减少退药的发生率，促进合理用药。

Objective To investigate the main reasons for drug repercussion in the outpatient pharmacy to ensure drug safety, reduce the incidence of irrational drug repercussion.Methods All the drug repercussion in the outpatient pharmacy from January 2014 to November 2014 in a hospital, were collected in order to analyze the reasons and types of drug repercussion, then relative measures were taken to intervene from January 2015 to November 2015.Results Compared to pre-intervention, the number of drug repercussion was reduced from 870 to 530, the reasons of drug repercussion were changed, there was no change in the types of drug repercussion.Conclusion Through the analysis of drug repercussion, some measures were taken to effectively reduce the incidence of drug repercussion, promote rational drug use.
