摘要
目的探究枸橼酸咖啡因与氨茶碱在早产儿原发性呼吸暂停中的应用效果及安全性。方法回顾性分析我院2014年1月-2016年1月收治的呼吸暂停早产儿共121例,依据应用药物的不同将其分为对照组与观察组。对照组接受氨茶碱治疗;观察组接受枸橼酸咖啡因治疗。比较两组患儿呼吸暂停治疗效果、药物性价比、不良反应发生率等指标。结果观察组呼吸暂停治疗总有效率为88.5%,明显高于对照组的73.3%,差异有统计学意义(P〈0.05)。观察组辅助呼吸支持应用率为63.9%,明显低于对照组的83.3%(P〈0.05)。观察组呼吸支持时间及甲基黄嘌呤类药物应用时间明显短于对照组(P〈0.05)。观察组住院花费明显少于对照组(P〈0.05)。观察组治疗期间不良反应发生率为9.8%,明显低于对照组的28.3%(P〈0.05)。结论枸橼酸咖啡因治疗早产儿呼吸暂停效果优于氨茶碱,可降低早产儿呼吸支持应用率,缩短呼吸支持时间及用药时间,降低住院花费,且安全性高于氨茶碱,早产儿不良反应发生率较低,值得临床推广应用。
Objective To explore the application effect and safety of caffeine citrate and aminophylline on primary premature apnea. Methods A retrospective analysis was conducted in our hospital from January 2014 to January 2016,121 cases of premature infants with apnea were divided into control group and observation group according to the different drugs.The control group received aminophylline treatment,the observation group received caffeine citrate treatment,the treatment effect and drug price,adverse reaction rate and other indicators were compared between two groups of children with respiratory pause. Results The treatment of total effective rate of apnea of prematurity in observation group was 88.5%,which was significantly higher than that of the control group(73.3%),the difference was statistically significant(P〈0.05).The use of respiratory supports rate in observation group was 63.9%,significantly lower than the control group′s respiratory support rate of 83.3%(P〈0.05).The ventilation time and methylxanthine therapy time in observation group was shorter than that of the control group(P〈0.05).The hospitalization cost in observation group was significantly less than that of the control group(P〈0.05).The observation group of adverse reactions during the treatment the incidence rate was 9.8%,significantly lower than the 28.3% of the control group(P〈0.05). Conclusion Caffeine citrate treatment of premature infants with respiratory pause effect is better than that of aminophylline,can reduce premature infants with respiratory support the use of rate and time,shorten the treatment time and reducing hospitalization cost,in addition,caffeine citrate safety is higher than that of aminophylline,premature infants with low incidence of adverse reactions,it is worth of clinical application.
