Efficacy and safety of Solitaire stent in acute thromboembolism events during endovascular treatment in patients with ruptured intracranial aneurysm

摘要 目的探讨Solitaire支架在处理颅内动脉瘤介入栓塞术中急性血栓事件的有效性及安全性。方法分析2009年7月至2015年1月8例行颅内动脉瘤介入栓塞术中发生急性血管闭塞患者,其中6例患者使用Soliraire支架行机械取栓,2例患者行Soliraire支架重建血管血流再通,并完成颅内动脉瘤致密栓塞。分析即时血管再通效果,术后出血情况及缺血情况。结果 8例患者均安全、有效地完成闭塞血管血流再通,6例患者机械取栓成功,2例患者多次取栓均未成功,考虑闭塞血管为斑块堵塞可能,予以支架覆盖斑块重建血管使血流再通;8例患者术后均未出现颅内出血及缺血征象,未见明显神经功能缺失。结论颅内动脉瘤血管内治疗术中急性栓塞事件发生时,Solitaire支架可通过机械取栓及支架重建血管方式能安全、有效地使血流再通。 Objective To investigate the efficacy and safety of Solitaire stent in acute thromboembolism events during endovascular treatment of ruptured intracranial aneurysm. Methods From July 2009 to January 2014,there were 8 patients with acute thromboembolism occurring during embolization therapy for intracranial aneurysm. Six patients immediately received mechanical thrombectomy using Solitaire stent,and the rest 2 patients received vascular reconstruction using Solitaire stent and finished dense embolization of intracranial aneurysms. Results In 8 patients with thromboembolism events,successful mechanical thrombectomy was performed on 6 patients. The rest 2 patients failed in thrombectomy for several times,and considering plaque blockage probability,revascularization was finished using Solitaire stent to promote blood flow recanalization. After operation,the 8 patients had no intracranial hemorrhage and ischemia phenomena,and no obvious neurological deficits were observed. Conclusion When acute thromboembolism events occurred in endovascular treatment of intracranial aneurysm,Solitaire stent can provide safe and effective blood flow recanalization by mechanical thrombectomy and stent revascularization.
