2型糖尿病患者血清25-羟基维生素D浓度变化及意义 被引量:6

摘要 目的探讨2型糖尿病(T2DM)患者血清25-羟基维生素D(VitD)水平的变化,以及VitD水平在T2DM患者体内与其他代谢指标的相关性。方法选择2014年3~9月在该院T2DM门诊及住院的T2DM患者65例(T2DM组)及糖耐量受损患者36例(糖耐量受损组),另选取同期年龄匹配的健康人为对照组(n=53),留取各组血清标本,测定VitD、血糖、胰岛素及血脂水平。结果糖尿病组与对照组VitD、餐后2小时胰岛素(2hIRI)、空腹血糖(FPG)、餐后2小时血糖(2hPG)及三酰甘油(TG)水平比较,差异均有统计学意义(P〈0.05),但两组IRI、胆固醇(TC)、高密度脂蛋白(HDL)及低密度脂蛋白(LDL)水平比较,差异均无统计学意义(P〉0.05);T2DM组与糖耐量受损组2h IRI、FPG、2hPG及TG水平比较,差异均有统计学意义(P〈0.05);糖耐量受损组与对照组VitD、2hIRI、2hPG、TG水平比较,差异均有统计学意义(P〈0.05)。结论 T2DM患者与糖耐量受损患者的糖代谢异常发生与血清VitD水平降低有关,血清VitD与体内脂代谢有关。 Objective To investigate the changes of serum 25-OH vitamin D(Vit D) level in the patients with type 2 diabetes mellitus(T2MD) and the relationship between the Vit D level with other metabolic indicators in T2 DM patients. Methods Sixty-five outpatients and inpatients with primary T2DM(T2DM group),36 patients with impaired glucose tolerance(impaired glucose tolerance group) and meanwhile 53 age-matched healthy persons(control) were randomly screened out in our hospital from March to September 2014. The serum samples were taken for determining vitamin D,blood glucose,insulin and blood lipid.Results The Vit D,2hIRI,FBG,2hPG and TG levels had statistical differences between the diabetic group and the control group(P 〈0.05);but the levels of IRI,TC,HDL and LDL had no statistical differences between these two groups(P 〈0.05);the levels of2 hIRI,FPG,2h PG and TG had statistical differences between the T2DM group and the impaired glucose tolerance group(P 〈0.05);the levels of Vit D,2hIRI,2hPG and TG had statistical differences between the impaired glucose tolerance group and the control group(P 〈0.05). Conclusion The glucose metabolism abnormality occurrence in the patients with T2DM and the patients with impaired glucose tolerance is related with the serum Vit D level decrease,moreover serum Vit D is related with the in vivo lipid metabolism.
