针刺治疗强直性脊柱炎临床疗效评价分析

Clinical curative effect evaluation of acupuncture on ankylosing spondylitis

目的:通过对针刺治疗强直性脊柱炎(AS)临床疗效分析,证明中医传统疗法在治疗本病方面的优势,同时为临床提供一种更为简便效廉的针刺方法。方法:收集40例肾虚湿热型AS患者,随机分为治疗组26例,对照组14例。两组患者均采用中药汤剂作为基础治疗,治疗组在基础治疗的方法上加上针刺疗法。观察治疗前后患者临床症状、实验室指标变化情况。结果:针刺疗法治疗AS具有较好的疗效,其ASAS20疗效达到76.9%,较对照组57.1%有显著差异(P<0.05),其ASAS40达到34.6%,也优于对照组21.4%,但差异无统计学意义。针刺疗法治疗AS在改善患者的临床症状及实验室指标方面均优于治疗前(P<0.05)。两组患者治疗后实验室指标比较,治疗组在Bath AS病情活动指数(BASDAI)、Bath AS功能指数(BASFI)和病人的总体评价(PGA)VAS评分方面均优于对照组(P<0.05)。结论:针刺治疗AS疗效突出,方法简便,临床更易于推广。

Objective： To prove the advantages of TCM on ankylosing spondylitis（AS） treatment through clinical effectiveness evaluation of acupuncture on AS treatment, and to provide a simple, effectiveness acupuncture method for practice. Methods： Forty cases of AS patients with syndrome of kidney deficiency and dampness-heat were collected and randomly divided into the treatment group（26 cases） and control group（14 cases）. The control group accepted Chinese medicine decoction by oral, and the treatment group accepted acupuncture therapy on the basis of Chinese medicine decoction. The changes of TCM symptoms and laboratory indexes were observed before and after treatment. Results： The ASAS20 efficacy in the treatment group reached 76.9%, with significant difference when compared with the control group（P〈0.05）, and the ASAS40 in the treatment group reached 34.6% that was also superior to the control group. Meanwhile, both of the clinical symptoms and laboratory indexes after the acupuncture treatment were superior to before treatment（P〈0.05）. In comparison of laboratory indexes, the scores of BASDAI, PGA and BASFI in the treatment group were all superior to those in the control group（P〈0.05）. Conclusion： Acupuncture has prominent effect on AS, with the simple, easy popularization advantages, so it is really worth promoting in practice.
