摘要
目的探讨Toll样受体1(TLRl)基因多态性及幽门螺杆菌感染与胃癌的相关性。方法选取2013年1月至2014年12月青岛大学医学院附属青岛市市立医院诊断的新发本地胃癌患者225例为观察组;同期选取健康查体的225例慢性胃炎患者为对照组,用测序方法检测2组患者TLR1基因rs4833095位点的基因型。采用多因素Logistic回归模型调整年龄、性别、幽门螺杆菌感染等因素后,分析TLR1基因多态性及幽门螺杆菌感染与胃癌的相关性。结果TLR1基因rs4833095位点GG、AG、AA的基因型频率在观察组中分别为33.8%(76/225)、49.8%(112/225)、16.4%(37/225),在对照组分别为43.6%(98/225)、48.0%(108/225)、8.4%(19/225),各基因型频率在2组间的分布差异有统计学意义(x2=8.64,P=0.013)。多因素Logistic回归显示,以GG基因型为参照,携带GA+AA基因型者发生胃癌的风险明显增加(比值比=1.42,95%置信区间:0.96~2.13)。联合效应分析显示,以携带rs4833095位点GG基因型且未感染幽门螺杆菌者为参照,携带GA+AA基因型并感染幽门螺杆菌者发生胃癌的风险明显增加(比值比=2.23,95%置信区间:1.28—3.86)。结论TLR1基因rs4833095位点多态性与胃癌发病风险密切相关。
Objective To investigate the correlations of polymorphisms of Toll-like receptor 1 (TLR1), helicobacter pylori(Hp). Methods Totally 225 patients with gastric carcinoma diagnosed for the first time (observa- tion group) and 225 patients with chronic gastritis (control group) from January 2013 to December 2014 were enrolled. The rs4833095 polymorphisms of TLR1 were genotyped by the gene sequencing. The correlations of poly- morphisms of TLR1, Hp infection and gastric cancer were analyzed after adjusting the age, gender, Hp infection with multiariable Logistic regression models. Results The frequencies of GG, AG and AA genotype on TLR1 rs4833095 were33.8% (76/225), 49.8% (112/225)and 16.4%(37/225)in observation group, 43.6% (98/225), 48.0% (108/225) and 8.4% (19/225) in control group, they were significantly different between groups (X2 = 8.64, P = 0.013). Compared with GG genotype carriers, GA + AA genotype carriers had significantly higher risk of gastric cancer ( odds ratio = 1.42, 95% confidence interval : 0. 96-2.13 ) . Joint effect analysis showed that compared with GG genotype carriers without Hp infection, GA + AA genotype carriers with Hp infection had significantly higher risk of gastric cancer ( odds ratio = 2.23, 95% confidence interval : 1.28-3.86). Conclusion Polymorphism of TLR1 rs4833095 is correlated with gastric cancer, a alleles have increased susceptibility to gastric cancer.
