摘要 目的探讨超高场强多源磁共振颈动脉成像技术对颈动脉病变的显示能力及优势。方法选取2015年2—6月北京怀柔医院颈动脉超声提示颈动脉内膜增厚及斑块形成患者35例,以3.0T磁共振多源成像技术配合颈动脉专用线圈对患者行“亮血”三维时间飞跃法(3D—TOF)磁共振血管成像(MRA)和“黑血”T1加权成像(T,WI)、T2加权成像(T2WI)、三维磁化强度预备梯度回波序列(3DMP—RAGE)检查,进行颈动脉狭窄程度和病变的分类,对比“亮血”3D—TOFMRA和“黑血”T1WI、T2WI、3DMP—RAGE图像检测结果,并评价图像质量。结果35例患者全部完成检查,共显示31例颈动脉狭窄。“亮血”和“黑血”序列扫描总时长(19.0±1.7)min。“亮血”3D—TOFMRA显示颈动脉血管腔无狭窄4例(“黑血”序列扫描显示斑块形成2例),轻度狭窄15例(“黑血”序列扫描显示斑块形成7例),中度狭窄11例(“黑血”序列扫描显示斑块形成1l例),重度狭窄4例(“黑血”序列扫描显示斑块形成4例),闭塞1例(“黑血”序列扫描显示斑块形成1例)。图像质量方面,视野内颈动脉区均未出现抗电效应导致的阴影,感兴趣区斑块结构均显示清晰。结论3.0T磁共振多源成像技术图像质量好,采用3D—TOF“亮血”结合“黑血”序列扫描,能够有效显示颈动脉斑块并判断狭窄程度。 Objective To evaluate the capability of ultrahigh field intensity multi-source magnetic reso- nance imaging (MRI) in displaying carotid artery lesions. Methods Totally 35 patients with carotid atheroselerotie plaque indicated by carotid ultrasonography underwent 3.0T multi-source MRI equipped with carotid artery dedica- ted coil. The all the cases were classified based on the degree of carotid stenosis and the type of lesions. The "bright blood" three-dimensional time of-flight magnetic resonance angiography (3D-TOF MRA), the " black blood" T1 weighted imaging, T2 weighted imaging and three-dimensional magnetization prepared rapid acquisition gradient echo sequences were observed to classify the degree of carotid stenosis; the imaging quality was also assessed. Results Totally 31 patients showed carotid artery stenosis in MRL The examination duration of "bright blood" plus "black blood" sequence scanning lasted for (19.0 ± 1.7 ) rain. " Bright blood" in 3D-TOF MRA showed 4 cases of non-stenosis (2 cases showed atherosclerotic plaque in " black blood" imaging) , 15 cases of slight stenosis (7 cases showed atheroselerotic plaque) , 11 cases of medium stenosis (11 cases showed atheroscle- rotic plaque) , 4 cases of severe stenosis (4 cases showed atherosclerotic plaque) and 1 case of occlusion (1 case showed atherosclerotic plaque). In the aspect of imaging quality, no dielectric effect artifact was observed and the plaque structure was clear. Conclusion The imaging quality of 3.0T multi-source MRI of carotid artery imaging is good, the atherosclerotic plaque and the degree of carotid artery stenosis can be effectively showed combined with "bright blood" 3D-TOF sequence and "black blood" sequence.
