CRP和SF检测在急性胰腺炎临床分型及严重程度评估中的意义

Significance of C reactive protein and ferritin in the evaluation of clinical classification and severity for acute pancreatitis

目的研究C反应蛋白(CRP)、铁蛋白(SF)的表达水平在急性胰腺炎(AP)临床分型及严重程度评估中的意义。方法收集该院2014年2月至2015年10月收治的AP患者120例,进行回顾性分析,其中重症急性胰腺炎(SAP)32例,轻型急性胰腺炎(MAP)88例,另选取30例健康体检者作为健康对照组。测定健康对照组和AP患者入院初的血清SF、CRP、淀粉酶(AMY)及脂肪酶(LPS)水平。结果与健康对照组比较,早期SAP组、MAP组的SF、CRP、AMY、LPS水平均明显升高(P<0.05);SAP组的SF、CRP水平均高于MAP组(P<0.05),而AMY、LPS水平与MAP组比较差异无统计学意义(P>0.05);SAP组SF阳性率水平明显高于MAP组(P<0.05);早期AP患者血清SF和CRP呈正相关(r=0.227,P=0.013)。结论 SF和CRP均能够作为急性胰腺炎患者分型的重要标志物,SF在判断患者严重程度上优于CRP,具有重要的临床应用价值。

Objective To study the significance of C-reactive protein（CRP）and ferritin（SF）in the evaluation of clinical classification and severity for acute pancreatitis（AP）.Methods The data of 120 patients with AP who selected in hospital from February 2014 to October 2015 randomly were analyzed,among them,32 cases of severe acute pancreatitis（SAP）,88 cases of mild acute pancreatitis（MAP）,30 healthy subjects were chosen as health control group.Serum levels of SF,CRP,amylase（AMY）and lipase（LPS）were measured in healthy controls and AP patients for the first time.Results Compared with healthy control group,the levels of SF,CRP,AMY,LPS were significantly elevated in early SAP group and MAP group（P〈0.05）.The levels of SF and CRP in SAP group were higher than those in MAP group（P〈0.05）,while the levels of AMY and LPS was not significantly different from the MAP group（P〈0.05）.The positive rate of SF in SAP group was significantly higher than that in MAP group（P〈0.05）;SF and CRP of early AP patients were positively correlated（r=0.227,P=0.013）.Conclusion SF and CRP are good indicators for early diagnosis of AP,and SF is more valuable in the differential diagnosis of MAP and SAP.
