经尿道1470nm半导体激光汽化术治疗良性前列腺增生症

The treatment of benign prostatic hyperplasia through 1470 nm semi-conductor laser mediated transurethral prostate vaporization

目的探讨经尿道1 470nm半导体激光汽化术治疗良性前列腺增生症(BPH)的疗效。方法收集72例BPH患者,随机分为36例应用1 470nm半导体激光行经尿道前列腺激光汽化术的BPH患者(激光组)和36例接受经尿道前列腺电切术的BPH患者(电切组),比较术中出血量、手术时间、术后导尿时间;同时对比两组患者术前及术后3个月国际前列腺症状评分(IPSS)、生活质量评分(QOL)、膀胱残余尿量及最大尿流率(Qmax)。结果两组患者手术均成功,激光组术后导尿时间是(3.18±1.04)d,手术时间(63.73±6.56)min;电切组术后导尿时间是(4.05±1.08)d,手术时间(72.88±12.56)min。激光组的术中出血明显少于电切组,差异具有显著性,两组均未发生再次导尿或二次手术事件。两组患者术后3个月,QOL评分、IPSS评分和Qmax与术前相比均有明显改善。结论 1 470nm半导体激光行经尿道前列腺激光汽化术可有效治疗BPH,术中、术后出血少,术后导尿时间短,效果肯定,并发症少,值得临床推广应用。

Objective To investigate the treatment of 1 470 nm semi-conductor transurethral vaporization of the prostate for benign prostatic hyperplasia（BPH）evaluated the efficacy of laser.Methods Totally 72 cases of BPH patients were collected,and randomly divided into two groups with 36 cases in each group.One group applied the 1 470 nm laser mediated transurethral vaporization of the prostate（laser group）.The other group underwent transurethral resection surgery（transurethral group）.The intra-operative blood loss,operation time and the postoperative catheterization were compared between the two groups.The international prostate symptom score（IPSS）,quality of life score（QOL）,post-void residual and maximum urine flow rate（Qmax）were recorded in three different time points of the two group patients,the time before surgery and three months after operation followed-up.Results The operations were successful in all the patients.The postoperative catheterization time in the laser group was（3.18±1.04）d and the operation time was（63.73±6.56）min;while in the transurethral group,the catheterization time was（4.05±1.08）d,the operation time was（72.88±12.56）min.The IPSS,QOL and Qmax were significant improved in the two groups.No re-catheterization or re-operation was occurred in the two groups.Conclusion Compared with traditional treatment,1 470 nm laser transurethral vaporization of the prostate can be more effective and safer therapy for the BPH.It has less blood loss in the intra-operative and post-operation,less catheterization time,less complications.It is worth to be further apply in the clinical.
