抗病毒软胶囊辅助阿昔洛韦治疗单纯疱疹病毒性角膜炎临床分析

The clinical analysis of adjuvant therapy of antiviral soft capsule in patients of simplex keratitis treated with acyclovir

目的探讨抗病毒软胶囊辅助阿昔洛韦治疗单纯疱疹病毒性角膜炎的临床疗效及安全性。方法选取2013年1月至2015年2月于重庆市丰都县人民医院接受治疗的107例单纯疱疹病毒性角膜炎患者为研究对象,按照治疗方案分为对照组52例62眼和观察组55例70眼。对照组给予阿昔洛韦滴眼液治疗,观察组在对照组基础上给予抗病毒软胶囊辅助治疗。疗效结束后,比较对照组和观察组患者临床疗效、视力情况、血清细胞因子水平、复发率及不良反应发生情况。结果观察组患者治疗总有效率明显高于对照组（94.3%vs.82.2%,P〈0.05）。治疗后,观察组患者中视力≥0.8的眼数比例明显多于对照组,差异有统计学差异（P〈0.05）,而两组患者治疗后视力≤0.4、〉0.4-〈0.8的眼数比例比较差异无统计学意义（P〉0.05）。与对照组相比,观察组患者治疗后血清白细胞介素（IL）-2水平明显升高,IL-6、干扰素（IFN）-γ水平明显降低,组间比较差异均有统计学意义（均P〈0.05）。随访6个月,观察组患者复发率明显低于对照组（7.1%vs.19.4%,P〈0.05）。治疗期间,两组患者均未出现严重不良反应,且对照组和观察组患者不良反应发生率比较差异无统计学意义（11.5%vs.16.4%,P〉0.05）。结论抗病毒软胶囊辅助阿昔洛韦治疗单纯疱疹病毒性角膜炎疗效可靠,能够明显提高临床疗效,改善视力,调节血清IL-2、IL-6、IFN-γ等细胞因子水平,降低复发率,且不良反应轻微,临床上值得应用。

Objective To investigate the curative efficacy and safety analysis of adjuvant therapy of antiviral soft capsule in patients of herpes simplex keratitis treated with acyclovir.Methods Totally 107 patients with herpes simplex keratitis who received therapy in our hospital from January 2013 to February 2015 were selected as research objects.According to therapeutic schemes,those patients were divided into the control group（n=52）with 62 eyes and the observation group（n=55）with 70 eyes.The control group was given treatment of acyclovir eye drops.Besides that,the observation group was given adjuvant therapy of antiviral soft capsule.After the course of treatment,the curative efficacy,visual acuity,level of serum cytokines,recurrence rate and adverse reactions were compared.Results The observation group had a total therapeutic efficacy ratio of 94.3%,which was statistically higher than that of 82.2%in the control group（P〈0.05）.After the treatment,in comparison with the control group,the observation group had statistically more cases with visual acuity≥ 0.8（P〈0.05）,while cases with visual acuity≤0.4and0.4-0.8were statistically same（P〉0.05）.As to cytokines,the observation group had statistically higher level of IL-2and lower levels of IL-6,IFN- γ（P〈0.05）.During the six-month follow-up,the recurrence rate in the observation group was 7.1%,which was statistically lower than that of 19.4%in the control group（P〈0.05）.During the treatment,there′s no case of severe adverse reaction in the two groups,and the incidence of adverse reactions in the control group and the observation group was statistically same（11.5%vs.16.4%,P〉0.05）.Conclusion Adjuvant therapy of antiviral soft capsule in treating herpes simplex keratitis with acyclovir is effective,and it can significantly increase curative efficacy,improve visual acuity,regulate levels of serum cytokines（IL-2,IL-6,IFN-γ）and reduce recurrence rate with minor adverse reactions.
