延续护理对乳腺癌保乳术后患者心理及生存质量的影响

EFFECT OF CONTINUITY OF CARE ON PSYCHOLOGICAL AND LIFE QUALITY OF PATIENTS WITH BREAST CANCER AFTER BREAST CONSERVING SURGERY

目的分析研究延续护理在乳腺癌保乳术中的应用体会以及对患者心理及生存质量的影响。方法选取108例行乳腺癌保乳术患者,在患者知情同意下采用骰子法将其分为观察组和对照组二组(各54例);在院治疗期间二组均实施护理干预,出院后对照组未给予任何护理干预,观察组实施延续护理;比较二组患者出院时、出院后3个月患者焦虑(SAS)、忧郁(SDS)、生活质量、运动功能评分变化。结果出院后3个月二组患者SAS、SDS、生存质量及FIM评分均有一定变化,组间比较出院后1、2、3个月观察组SAS、SDS均显著低于对照组(P<0.05),生存质量及FIM评分均显著高于对照组(P<0.05)。结论延续护理的实施能进一步提升乳腺癌保乳术患者对疾病及康复疗养相关知识的认知度,进而增强自我防护及并发症防治,改善心理状况和生存质量,对患者康复疗养具有重要的意义。

Objective to analyze and study the application of continuity of care in breast cancer patients with breast conserving surgery.Methods A total of 108 cases of breast cancer patients with breast conserving surgery,the patients＇ informed consent in the dice method were divided into the observation group and the control group（54 cases in each）.In the hospital during the period of treatment in the two groups are the implementation of nursing intervention,patients in control group were not given any nursing intervention,observation group implementation of continuity of care;compared two groups of patients discharged from hospital after discharge,three months of two groups of patients with anxiety,depression,quality of life,motor function score changes.Results Discharged after three months of two groups of patients with SAS,SDS,quality of life and FIM scores were some changes,between the two groups after discharge were compared at 1,2 and 3 months of observation group of SAS and SDS were significantly lower than those in the control group（P〈0.05）,quality of life and FIM scores were significantly higher than those of the control group（P〈0.05）.Conclusions The continue nursing implementation can further enhance breast cancer breast conserving surgery in patients with knowledge of the disease and rehabilitation awareness,thus enhancing the self-protection,prevention and treatment of complications,improve the psychological status and quality of life,and on the rehabilitation of patients with convalescent has important significance.
