利多卡因复合舒芬太尼硬膜外麻醉在剖宫产术中的应用效果研究

目的探究利多卡因复合舒芬太尼硬膜外麻醉在剖宫产术中的应用效果。方法选取我院2014年5月至2015年5月产科收治的122例剖宫产产妇为研究对象,采取随机数字表法将所有产妇分为研究组(61例)与对照组(61例),对照组产妇予以利多卡因硬膜外麻醉,研究组患者予以利多卡因复合舒芬太尼硬膜外麻醉,观察对比两组产妇麻醉效果、麻醉起效时间、镇痛持续时间及麻醉诱导至胎儿娩出时间、新生儿Apgar评分及麻醉不良反应情况。结果研究组产妇麻醉效果总优良率为98.36%,对照组为86.89%,研究组总优良率明显高于对照组,P<0.05,差异具有统计学意义;研究组产妇麻醉起效时间及麻醉诱导至胎儿娩出时间明显短于对照组,而镇痛持续时间明显比对照组长,差异具有统计学意义(P均<0.05);两组新生儿Apgar评分及麻醉总不良反应率相当,对比无统计学差异(P>0.05)。结论利多卡因复合舒芬太尼硬膜外麻醉在剖宫产术中的应用效果良好,麻醉效果佳,麻醉起效快,镇痛时间长,不良反应少,对新生儿影响小,安全可行,具有较高临床推广及应用价值。

Objective To explore the application effect of epidural anesthesia by composite sufentanil and lidocaine in cesarean section. Methods From May 2014 to May 2015,122 cases of cesarean section maternal recorded in obstetric department of our hospital were selected as the research object,take a random number table method all women were divided into research group（ 61 cases） and control group（ 61 cases）.Maternal in control group received lidocaine epidural anesthesia,patients in research group were given lidocaine combined sufentanil for epidural anesthesia. Anesthesia effect,working time,duration of analgesia and time from anesthesia induction to fetal childbirth,anesthetic adverse reactions and neonatal Apgar score of the two groups were compared. Results The eximious rate of anesthesia effect in research group was 98. 36%,it was 86. 89%in the control group,the difference was statistically significant（ P 0. 05）. Team work time and the time from anesthesia induction to fetal childbirth of study group was significantly shorter than the control group,but analgesia duration was longer than that of contrast group obviously,the differences were statistically significant（ P 0. 05）. Neonatal Apgar score and total anesthesia adverse reaction rate of the two groups had no statistical difference（ P 0. 05）. Conclusions Lidocaine combined with sufentanil epidural anesthesia in cesarean section obtained good application effect,with the advantages including good anaesthesia effect,fast anesthesia work,long analgesic time,less adverse reaction,small impact on neonatal,safe and feasible,so it has high clinical popularization and application value.
