微小R-125b在初发急性髓系白血病中的表达情况及临床意义 被引量:7

Expression and clinical significance of minimal R-125b in primary acute myeloid leukemia

摘要 目的分析微小R-125b(miR-125b)在初发急性髓系白血病(AML)中的表达情况及临床意义。方法选取初发急性髓系白血病患者48例和非恶性血液病患者20例作为对照组,采用实时荧光定量PCR检测骨髓细胞中的miR-125b表达水平。整理患者的临床资料,追踪初发急性髓系白血病患者的治疗效果。结果在初发AML的不同亚型中,M3的miR-125b表达水平高于其他亚型,差异有统计学意义(P<0.05)。初发患者、难治复发患者及化疗缓解患者miR-125b表达水平明显高于对照组,差异有统计学意义(P<0.05)。PML/RARA融合基因阳性的初发APL患者的miR-125b表达水平显著高于PML/RARA融合基因阴性患者,差异具有统计学意义(P<0.01)。初发AML患者miR-125b表达水平与骨髓原始幼稚细胞呈正相关(r=0.773,P<0.05)。结论 miR-125b水平在急性髓系白血病患者中存在着过度表达,其表达水平与疾病状态有关。通过连续监测PML/RARA融合基因阳性者的miR-125b表达水平,预测miR-125b也许可作为急性早幼粒细胞白血病治疗预后分子。 Objective To analyze the expression and clinical significance of minimal R- 125b( miR- 125b) in primary acute myeloid leukemia( AML). Methods To select 48 cases of primary AML and 20 cases of non malignant hematological diseases as control group,and the expression level of miR- 125 b in bone marrow cells were detected by real- time fluorescence quantitative PCR. To sort out patient's clinical data,and track the treatment effect in patient with primary AML. Results In different subtypes of primary AML,the expression level of miR- 125 b in M3 was higher than that in other subtypes,and the difference was statistically significant( P〈0. 05). The expression level of miR- 125 b in patient with primary,refractory and relapsed,chemotherapy remission AML were significantly higher than that of the control group,and the difference was statistically significant( P〈0. 05). The expression level of miR- 125 b in primary APL patients with positive PML / RARA fusion gene was significantly higher than that of in patients with negative PML / RARA fusion gene,and the difference was statistically significant( P〈0. 01). The expression level of miR- 125 b in patient with primary AML was positively correlated with blast and immature cells of bone marrow( r = 0. 773,P〈0. 05). Conclusion Primary AML patients have an over- expression level of miR- 125 b,and the expression level is related to the disease state. By continuous monitoring of the expression levels of miR- 125 b in patient with positive PML / RARA fusion gene,miR- 125 b prediction may serve as a prognostic judgment in the treatment of patients with acute myeloid leukemia.
