CD64、降钙素原和C-反应蛋白在学龄前儿童早期细菌感染性疾病中的应用 被引量:10

Analysis of the application of CD64,procalcitonin and C-reactive protein in preschool children with early bacterial infection diseases

摘要 目的探讨CD64、PCT和CRP的测定在学龄前儿童早期细菌感染性疾病中的诊断价值。方法选取2014年1月-2015年12月本院儿科病房感染患者,根据病原体检测结果分为细菌感染组和病毒感染组,以健康体检学龄前儿童作为正常对照组,分别检测各组研究对象CD64、PCT和CRP水平。结果细菌感染组与病毒感染组和正常对照组相比,CD64、PCT和CRP水平明显升高,差异有统计学意义(P<0.05);细菌感染组经有效治疗后,三者均显著降低,差异有统计学意义(P<0.05);细菌感染组CD64、PCT和CRP的阳性率分别为96.3%、91.3%和71.3%,明显高于病毒感染组和正常对照组,差异有统计学意义(P<0.05);CD64和PCT诊断细菌感染的敏感度、特异度、阳性预测值、阴性预测值均明显高于CRP,差异有统计学意义(P<0.05);相关性分析显示,细菌感染组CD64指数和PCT水平呈正相关(r=0.865,P<0.01)。结论 CD64、PCT和CRP联合检测可及时有效的诊断学龄前儿童早期细菌感染,其中CD64和PCT的诊断效率要优于CRP。 Objective To investigate the diagnostic value of the CD64,Procalcitonin and C- reactive protein in preschool children with early bacterial infection diseases. Methods Infected patients were selected with pediatric ward in our hospital from January 2014 to December 2015,and according to the pathogen detection results,which were divided into the bacterial infection group and the virus infection group,and healthy preschool children were taken as normal control group. The CD64,Procalcitonin and C- reactive protein in the peripheral blood of each group were detected. Results The levels of CD64,PCT and CRP in the bacterial infection group were significantly higher than that in the viral infection group and the normal control group,with the difference statistically significant( P〈0. 05); after effective treatment in bacterial infection group,CD64,PCT and CRP significantly reduced,with the difference statistically significant( P〈0. 05); the positive rates of CD64,PCT and CRP in bacterial infection group were 96. 3%,91. 3% and 71. 3%,respectively,which was significantly higher than the virus infection group and the normal control group,with the difference statistically significant( P〈0. 05); the sensitivity,specificity,positive predictive value and negative predictive value of CD64 and PCT in the diagnosis of bacterial infection were significantly higher than that of CRP,with the difference statistically significant( P〈0. 05); correlation analysis showed that CD64 and PCT levels in the bacterial infection group were positively correlated( r = 0. 865,P〈0. 01). Conclusion The combined detection of CD64,PCT and CRP can effectively diagnose the early bacterial infection in preschool children,and the diagnostic efficiency of CD64 and PCT was better than CRP.
