CAD/CAM一体化纤维桩核与全瓷冠即刻修复重度缺损前牙的2年临床观察 被引量:10

Two-year clinical research on immediate restoration of severe defect anterior teeth by CAD/CAM one-piece fiber post-and-core and all ceramic crowns

摘要 目的评价使用一体化纤维桩核与玻璃陶瓷全瓷冠即刻修复根管治疗后的重度缺损前牙的临床效果。方法选择严重缺损经根管治疗后需桩核冠修复的上颌前牙,共81例患者的89颗牙。采用初次就诊即刻制作CAD/CAM个性化一体化玻璃纤维桩核与IPS e.max CAD全瓷冠的方式对患牙进行修复。修复完成后观察24个月,对修复成功率、留存率进行描述,记录失败模式并分析原因。结果观察期结束时,所有修复体都维持稳定状态并功能良好,存留率为100%。其中1例尖牙全瓷冠发生牙尖轻微折裂,4例全瓷冠边缘出现密合度欠佳,并有轻微着色,但无继发龋发生,因此成功率为94.4%。结论限于此短期观察,CAD/CAM个性化一体化纤维桩核结合IPS e.max CAD全瓷冠即刻修复严重缺损的上颌前牙效果良好。 Objective To investigate the 2-year clinical performance of CAD/CAM one-piece fiber post-andcore and all ceramic crowns in immediate restoration of severe defect anterior teeth. Methods Eighty-nine endodontically treated severe defect anterior teeth of 81 patients were brought into investigation. All teeth were restorated by CAD/CAM custom one-piece glass fiber post-and-core and IPS e.max CAD all ceramic crowns. At the end of the observation time,which was set as 2 years, the survival and success rate of the restorations as well as failure patterns and reasons would be analyzed. Results At the end of 2-year observation, all restorations were in stable state and functioned well, so the survival rate was 100%.Since a slightly cusp fracture happened to a canine ceramic crown, and discrepancy accompany with slight discoloration on the margin but without decay were detected on 4 crowns, success rate was 94.4%.Conclusion Within the limits of this short-term investigation, CAD/CAM one-piece custom fiber post-and-core combined with IPS e.max CAD all ceramic crowns seemd to be a very reliable method to immediate restoration of severe defect anterior teeth.
