成人后腹腔巨大占位的临床诊治体会(附14例报告)

Clinical experience in diagnosis and therapy of adults huge retroperitoneal lesions(report of 14 cases)

目的探讨成人后腹腔巨大占位的临床特点和诊治方法.方法回顾性分析近4年我科收治的1 1 例成人后腹腔巨大占位患者的临床资料,总结其诊断方法、围手术期准备与手术治疗方式.结果 1 4 例中1 1 例为良性肿瘤,其中9 例完全切除, 2 例行肿瘤大部切除术; 例为恶性肿瘤,其中2 例完全切除,1 例行肿瘤大部切除术.术后随访3 -4 6 个月,1 例恶性肿瘤复发,行二次手术切除并辅助治疗; 例行部分切除术的良性肿瘤稍有增大,等待观察;其余患者无肿瘤复发.结论良好的术前准备、联合脏器切除和术后辅助治疗可使大部分良性成人后腹腔巨大肿瘤获得治愈,恶性肿瘤获得缓解.

Objective To study the clinical characteristic and treatment of adults huge retroperitoneal lesions.Methods We analyzed the clinical data of 14 cases of adults huge retroperitoneal lesions in our department by the recent 4years retrospectively,and summarized the diagnostic methods,perioperative preparation and surgical treatment. Results Among 14 cases,11cases were benign tumors,of which 9cases were completely removed and 2cases underwent subtotal tumor resection,3cases were malignant tumors,2cases were completely resected,1case underwent subtotal resection of tumor.Postoperative follow up ranged from 3to 46 months,there were 1cases of malignant tumor recurrence,two surgical resection and adjuvant therapy,2cases of partial resection of benign tumor slightly increased,waiting for observation;the rest of the cases without tumor recurrence. Conclusions Effective pre-operation preparation,combined organ resection and postoperative adjuvant therapy can be helpful in the treatment of both benign and malignant adult huge retroperitoneal lesions.It can cure most benign tumors and relief the progress of malignant ones.
