摘要
目的探讨超声弹性成像组织弥散定量分析在军训小腿肌肉损伤中的诊断价值。方法选取军训致小腿肌肉损伤22例作为研究对象,以正常小腿肌肉作为自身对照,分析肌肉损伤与自身对应部位正常肌肉二维图像与弹性成像的图像特点;运用超声弹性成像组织弥散性定量分析软件评价肌肉组织的硬度,对照正常肌肉和肌肉损伤各弹性参数的变化。结果肌肉损伤组11个参数中,应变均值、蓝色区域所占百分比、复杂度较对侧均存在统计学意义(P<0.001),标准差、偏度、峰度、对比度、均等性、杂乱度、一致度、相关性无统计学意义(P>0.05),提示损伤后肌肉弹性应变均值减低、蓝色区域所占百分比及复杂度增加,肌肉硬度增加。结论超声弹性成像弥散性定量分析在二维超声的基础上能提供客观、准确的肌肉硬度的定量数据及图像信息,提高了肌肉局部损伤及小的肌纤维撕裂诊断敏感性,此技术运用于军训肌肉损伤诊断能为早期诊断肌肉损伤并指导康复训练提供客观依据。
Objective To investgate the value of ultrasonic elastography strain histogram in diagnosing the crus muscle injuring in military training. Methods A total of 22 objects from military training of which crus muscle injured as the subjects, the characteristics of Gray-scale sonography image and elasticity image were analyzed,and the self contralat- eral normal crus muscle as a control; compared the muscle hardness between experimental group and control group. Results Average relative strain value(MEAN), area ratio of low-strain region (AREA%) and complexity(COMP) had statistical differences in the eleven parameters of injury group when compared with the control group (P〈0. 001 ). The standard deviation, kurtosis, skewness, contrast, equivalence, disorder degree, consistent degree, correlation had no sta- tistical differences (P〉0. 05). The results suggested the reduction of MEAN, and increase of AREA% and COMP, the evidence of muscle hardness increased after the musles injuring. Conclusion On the basis of Gray-scale sonography, ul- trasonic elstography strain histogram quantitative analysis can provide objective and accurate quantitative data and image information of muscle hardness. This technology supplies an objective basis for diagnosing musles injury at its early stage and guiding rehabilitative training after, if it apply to muscle injury diagnosing during military training.
