超声造影术评价不同方式治疗输卵管妊娠的临床价值

Clinical value of ultrasound angiography in the evaluation of clinical effect of different ways in thetreatment of tubal pregnancy

目的探讨采用超声造影术对不同方式治疗输卵管妊娠效果的评估价值。方法随机选取2013年8月至2015年5月本院收治的输卵管妊娠患者120例，按照治疗方法的不同将其分为腹腔镜组、介入治疗组和保守治疗组，于治疗结束3～6个月后，对三组患者实施阴道四维输卵管超声造影术，并对患者患侧输卵管的畅通性进行观察记录，并同宫内妊娠结果进行对比。结果腹腔镜组患者输卵管畅通率（92．50％）明显高于介入治疗组（70．00％）和保守治疗组（55．00％），差异有统计学意义（P〈O．05）；腹腔镜组患者宫内妊娠率（62．50％）高于其他两组（47．50％、42．50％），但三组相比并无统计学差异（P〉0．05）。结论采用输卵管超声造影术对患者术后输卵管形态及通畅情况进行评估，可对治疗效果进行准确评价，对于宫内妊娠的预测具有十分积极的作用，建议在临床进一步推广使用。

Objective To explore the clinical value of ultrasound angiography in the evaluation of clinical effect of different ways in the treatment of tubal pregnancy. Methods 120 cases of tubal pregnancy admitted to our hospital from August 2013 to May 2015 were randomly selected, and divided into laparoscopic group, interventional treatment group, and conservative treatment group according to different treatment methods. 3 to 6 months after the end of treatment, patients of three groups underwent four-dimensional transvaginal tubal ultrasound angiography, and ipsilateral tubal patency was observed and recorded, and the results were compared with intrauterine pregnancy. Results The tubal patency rate of laparoscopic group was 92.50%, significantly higher than 70.00% ofinterventional therapy group and 55.00% of conservative treatment group, with statistically significant difference （P〈0.05）. Theintrauterine pregnancy rate of laparoscopic group （62.50%） was higher than those of other two groups （47.50%, 42.50%）, without statistically significant difference （P〉0.05）. Conclusion Ultrasound angiography in the evaluation of postoperative oviduct morphology and tubal patency can assess the therapeutic effect more accurately, which has very positive effect to predict intrauterine pregnancy, worthy of further clinical promotion.
