两种宫腔镜分离手术治疗宫腔粘连

Clinical effects of two types of hysteroscopic surgery on intrauterine adhesion

目的：探讨宫腔镜微型剪刀分离手术治疗宫腔粘连的临床效果，并与宫腔镜电切分离手术效果相比较。方法将103例宫腔粘连患者随机分为观察组52例和对照组51例，观察组采用宫腔镜微型剪刀切除术，对照组行宫腔镜电切分离术，比较两组手术时间、术中出血量、术中并发症情况，术后1、3个月宫腔再粘连发生率以及临床疗效。结果两组手术时间、术中出血量及并发症等比较差异未见统计学意义（P ＞0.05）；观察组术后1、3个月宫腔再粘连发生率为26.9％、13.5％，对照组为29.4％、17.6％，组间比较差异未见统计学意义（P ＞0.05）；术后3个月复查，观察组总有效率为90.4％，对照组为92.2％，组间比较差异未见统计学意义（ P ＞0.05）。结论宫腔镜微型剪刀分离手术治疗宫腔粘临床效果与宫腔镜电切分离手术相似，宫腔镜剪刀分离术临床应用简便，可门诊完成，适于基层医院推广应用。

Objective To investigate the clinical effects of hysteroscopic micro-scissors separa-tion surgery on intrauterine adhesion and compared the effects with the hysteroscopic electric resection. Methods One hundred and three patients with intrauterine adhesion were randomly divided into two groups with 52 patients in observation group and 51 patients in control group. The patients in observation group were treated with the hysteroscopic micro-scissors separation surgery and the patients in control group were treated with the hysteroscopic electric resection. The time of operation,amount of bleeding, intraoperative complications during the operation and the intrauterine adhesion re-occurrence rate and clinical effects 1 and 3 months after the operation in the two groups were compared. Results The time of operation,amount of bleeding and intraoperative complications of the patients in the two groups had no significant difference（P 〉 0. 05）. After 1 month and 3 months of the operation,the intrauterine adhesion reoccurrence rate of the patents in observation group was 26. 9% and 13. 5% ,and that of the control group was 29. 4% and 17. 6% ,there was no significant difference between the two groups（P 〉 0. 05）. Review of 3 months after the operation showed the total effective rate of the observation group was 90. 4%and that of 92. 2% in the control group,there was no significant difference between the two groups（P 〉0. 05）. Conclusions The clinical effect of hysteroscopic micro-scissors separation surgery on intrauter-ine adhesion is similar with the hysteroscopic electric resection. The hysteroscopic micro-scissors separa-tion surgery is easy to apply and can complete in outpatient service,so it is suitable for primary hospital to popularize and apply.
