射频消融术联合胶原酶髓核溶解术治疗巨大型腰椎间盘突出症

Effects of radiofrequency ablation combined with collagenase chemonucleolysis on huge lumbar disc herniation

摘要 目的:观察射频消融术联合胶原酶髓核溶解术治疗巨大型腰椎间盘突出症的临床效果。方法选取2012年9月至2014年9月60例巨大型腰椎间盘突出症患者作为研究对象,按照随机数字表法将其分为对照组与观察组,每组30例。对照组仅接受单一射频消融术治疗,观察组给予射频消融术联合胶原酶髓核溶解术治疗,比较两组患者的临床疗效。结果观察组优17例,良11例,可1例,整体优良率为96.7%,明显高于对照组的60.0%(P <0.05);观察组术后评分提升至(23.1±3.7)分,明显高于对照组的(18.2±3.0)分,组间对比差异有统计学意义(P <0.05)。结论在巨大型腰椎间盘突出症患者的临床治疗中,采用射频消融术联合胶原酶髓核溶解术方案可优化临床治疗效果,改善患者的临床症状,弥补了单一射频消融术治疗的不足,疗效显著,值得推广。 Objective To observe the clinical effects of radiofrequency ablation combined with collagenase chemonucleolysis on huge lumbar disc herniation. Methods Sixty cases of huge lumbar disc herniation from September 2012 to September 2014 were selected as the research objects and randomly di-vided into the control group and the observation group with 30 cases in each group. The control group re-ceived single radiofrequency ablation therapy merely,while the observation group was treated with radio-frequency ablation combined with collagenase chemonucleolysis. The clinical efficacy of the two groups were compared. Results There were 17 excellent cases,11 good cases and 1 effective case in the obser-vation group;the excellent and good rate was 96. 7% ,which was significantly higher than that of the control group(60% ,P 〈 0. 05);the postoperative score of the observation group increased to(23. 1 ± 3. 7),which was significantly higher than that of the control group(18. 2 ± 3. 0),and the difference was significant(P 〈 0. 05). Conclusions In the clinical treatment of patients with huge lumbar disc hernia-tion,the scheme of radiofrequency ablation combined with collagenase chemonucleolysis can optimize the clinical curative effects,improve the patient’s clinical symptoms,making up for the deficiency of single radio frequency ablation,and curative effect of the scheme is remarkable,so it is worthy of promotion.
