下颈椎单侧小关节脱位不同旋转角度对椎动脉的影响研究

摘要 目的研究下颈椎单侧小关节脱位不同旋转角度对椎动脉流量和长度变化的影响。方法 12具新鲜国人颈椎尸体标本作为实验材料,进行椎动脉灌注实验并测量小关节脱位交锁角度,观察脱位节段不同小关节旋转角度的椎动脉流量和血管长度,并在解剖形态学以及病理学上观察椎动脉的相关变化。结果关节脱位旋转5°时,脱位后以及复位后脱位侧椎动脉流量及长度与脱位前相比,差异均无统计学意义(均>0.05);而脱位旋转角度≥10°时,椎动脉的流量和长度改变差异均有统计学意义(均<0.05)。结论屈曲旋转外力导致颈椎单侧小关节脱位,由此产生的颈椎脱位形态改变可对椎动脉产生显著的压迫,并可能导致椎动脉结构不可逆转的改变。
