持续体内外循环胸腔热灌注化疗的疗效观察

Clinical observation of continuous body cavity and extracorporeal circumfusion chemotherapy for malignant pleural fluid

目的观察持续体内外循环热灌注联合化疗在治疗恶性胸腔积液的疗效。方法对确诊为恶性胸腔积液的48例肺癌、乳腺癌患者,先排尽胸腔积液,将体内外循环生理盐水或蒸馏水及化疗药升温至46℃,将其灌入胸腔内,使胸腔内温度恒温在44.5-45.5℃进行热疗,观察患者近期疗效。结果恶性胸腔积液明显减少或消失,呼吸困难、胸闷、咳嗽等症状明显好转,不良反应较小。结论持续体内外循环热灌注联合化疗对治疗恶性胸腔积液近期疗效确切,不良反应轻。

Objective The goal of this study is to investigate the treatment effects of continuous body cavity and extracorporeal circumfusion combined with chemotherapy for malignant pleural fluid. Methods For 48 patients with lung cancer and breast cancer diagnosed for malignant pleural fluid of pleural effusion,their pleural effusions were first drained,and then body cavity and extracorporeal circulation saline or distilled water,and chemotherapy drugs heated to 46℃ were poured into the chest,the temperature was maintained 44. 5℃ - 45. 5℃,short-term effect was further observed in patients. Results Malignant pleural fluids were distinctly disappeared or lessened. Moreover,clinical symptoms（ such as dyspnea,chest tightness,and cough） were significantly improved,and there were fewer side effects. Conclusion The continuous body cavity circumfusion combined with chemotherapy had a good treatment effects for malignant pleural fluids.
