摘要
目的:探讨冠心病合并高血压患者左室质量指数(LVMI)与冠脉病变严重程度的相关性。方法:选取在我院住院的冠心病合并高血压患者168例为研究对象,所有患者入院后均行心脏超声检查,按照LVMI水平的中位数将患者分为A组(LVMI≥115.71g/m^2,n=82例)和B组(LVMI<115.71g/m^2,n=86例);比较两组的实验室检查指标与冠脉病变支数及Gensini积分差异。利用多元线性回归方程分析影响Gensini积分的相关因素。结果:与B组比较,A组患者收缩压(SBP)、甘油三酯(TG)、B型脑钠肽(BNP)、血清肌酐(Scr)较高(P<0.05);A组中Gensini积分亦高于B组(P<0.05),三支病变的比例高于B组(P<0.05);Gensini积分高分组LVMI高于中分组及低分组(P<0.05),中分组高于低分组(P<0.05);Pearson相关性分析显示:Gensini积分与LVMI呈现正相关(P<0.05),亦与SBP、TG、Scr、BNP呈正相关(P<0.05);多元线性回归分析显示:LVMI是Gensini积分增高的独立危险因素(P<0.05)。结论:冠心病合并高血压患者LVMI增高与冠脉病变严重程度具有一定正相关性,可能是影响患者冠脉病变严重程度的独立危险因素。
Objective: To investigate the correlation between the left ventricular mass index(LVMI) and the severity of coronary artery lesions of patients with coronary heart disease plus hypertension. Methods:168 patients with coronary heart disease and high blood pressure were selected as the research objects. All patients after admission had heart ultrasound examination. According to the median level of LVMI, they were divided into group A(LVMI≥115.71 g/m^2, n = 82) and group B(LVMI 115.71 g/m^2, n = 86). Record the general clinical data of patients and related laboratory examination indexes, coronary lesions and Gensini score, and compare the differences between the two groups. Multiple linear regression equation was applied to analyze the related factors influencing the Gensini score. Results:The systolic blood pressure(SBP), and levels of triglycerides(TG), b-type brain natriuretic peptide(BNP), serum creatinine(Scr) were higher in group A than in group B(P〈0.05). Gensini score in group A was higher than in group B(P〈0.05). And the proportion of triple vessel lesions was higher in group A than in group B(P〈0.05). The LVMI was the highest in the group of high Gensini score(P〈0.05), and lowest in the group of low Gensini score(P〈0.05). Pearson correlation analysis showed that the Gensini score had positive correlation with LVMI(P〈0.05). It was also positively correlated with SBP, TG, Scr, BNP(P〈0.05). Multiple linear regression analysis showed that LVMI was an independent risk factor of the increased Gensini score(P〈0.05). Conclusion: The increase of LVMI had positive correlation with the severity of coronary artery lesions of hypertension coronary heart disease patients combined with high blood pressure. LVMI could be the independent risk factor of the severity of coronary lesions.
