Analysis of the Correlation of Circulating Follicular Helper T Cells Subsets with Clinical Features of Myasthenia Gravis

摘要 目的:分析循环滤泡辅助性T(c Tfh)细胞亚型与重症肌无力(MG)患者临床特点之间的关系。方法:横断面研究30例MG患者外周血c Tfh细胞各个亚型百分比和临床特点之间的关系。c Tfh细胞亚型的百分比是通过流式细胞术获取的。MG患者的临床特点包括性别、年龄、病程、胸腺情况、美国重症肌无力协会(MGFA)分型和重症肌无力量化(QMG)评分。结果:CD4^+CXCR5^+ICOS^+、CD4^+CXCR5^+PD-1^+和CD4^+CXCR5^+CXCR3-CCR6^+(Th17样)c Tfh细胞亚型百分比与QMG评分之间存在正相关关系;全身型重症肌无力(GMG)患者在CD4^+CXCR5^+ICOS^+、CD4^+CXCR5^+PD-1^+和Th17样c Tfh细胞亚型百分比要高于眼肌型重症肌无力(OMG)患者,OMG患者在CD4^+CXCR5^+CXCR3-CCR6-(Th2样)c Tfh亚型细胞百分比要高于GMG患者。c Tfh细胞各个亚型百分比与MG患者的性别、年龄、病程、胸腺情况均无显著相关性。结论:CD4^+CXCR5^+ICOS^+、CD4^+CXCR5^+PD-1^+和Th17样c Tfh细胞亚型百分比与MG病情严重性间存在正相关关系,提示c Tfh细胞与MG之间的密切关系。 Objective: To analyze the correlation of circulating follicular helper T(c Tfh) cells subsets with clinical features of myasthenia gravis(MG). Methods: The correlations of peripheral blood c Tfh cells subsets with clinical features of 30 cases of MG patients were studied cross-sectionally. Frequencies of the subsets of c Tfh cells were determined by flow cytometry. Clinical features included gender, age, disease duration, the situation of thymus, Myasthenia Gravis Foundation of America(MGFA) clinical classification and Quantitative Myasthenia Gravis(QMG) score. Results: There were positive correlations between the frequencies of CD4^+CXCR5^+ICOS^+,CD4^+CXCR5^+PD-1^+, CD4^+CXCR5^+CXCR3-CCR6^+(c Tfh-Th17) cells and the QMG scores. The frequencies of CD4^+CXCR5^+ICOS^+,CD4^+CXCR5^+PD-1^+, c Tfh-Th17 cells in generalized MG(GMG) were higher than those of ocular MG(OMG). The frequencies of c Tfh-Th2 cells in ocular MG(OMG) were higher than those of generalized MG(GMG). There was no correlation of frequencies of c Tfh cells subsets with gender, age, disease duration and the situation of thymus. Conclusions: There were positive correlations between the frequencies of CD4^+CXCR5^+ICOS^+, CD4^+CXCR5^+PD-1^+, c Tfh-Th17 cells and the severity of MG, which indicated the close relationship between the c Tfh cell and MG.
