健康体检时彩色多普勒超声诊断甲状腺癌的价值 被引量:12

Value of color Doppler ultrasound in the diagnosis of thyroid carcinoma in health examination

摘要 目的探讨彩色多普勒超声在健康体检中对甲状腺癌的诊断价值。方法选取2010年2月至2013年12月间在海南省万宁市人民医院进行健康体检时发现甲状腺结节的310例患者。均行超声检查和计算机断层扫描(CT)检查,比较超声检查与术后病理学检查结果,以及超声检查与CT检查的敏感性、特异性和准确率。结果 310例患者均经手术治疗,术后病理结果显示,226例为良性结节;84例为甲状腺癌,其中乳头状腺癌76例,滤泡性腺癌4例,髓样癌2例,低分化癌2例。超声诊断的特异性为68.1%,准确性为71.6%;CT检查的特异性为45.6%,准确性为52.9%,差异有统计学意义(P<0.05)。结论彩色多普勒超声检查能较准确诊断甲状腺良恶性结节。 Objective To evaluate color doppler ultrasound in the diagnosis of thyroid carcinoma.Methods From February 2010 to December 2013,310 patients of thyroid nodule were selected in Wanning city people's hospital for medical examination. All patients underwent ultrasonography and computed tomography( CT) examination,compare the results of ultrasound examination and postoperative pathological examination and comparison of ultrasound and CT examination of the sensitivity,specificity and accuracy. Results 310 patients after surgery,postoperative pathological results showed that 226 cases of benign nodules,84 cases of thyroid carcinoma. Of 84 cases of thyroid malignant tumor,76 cases of papillary adenocarcinoma,4 cases of follicular adenocarcinoma,2 cases of medullary carcinoma,2 cases poorly differentiated carcinoma. The specificity and accuracy of the ultrasound diagnosis were 68. 1%,71. 6%,significantly higher than that of CT examination,accuracy,specificity,were 45. 6%,52. 9%,the difference was statistically significant( P〈 0. 05). Conclusion Color doppler ultrasound can accurately diagnosis of benign and malignant thyroid cancer nodules,worthy of application.
