2型糖尿病伴特发性甲状旁腺功能减退症一例

Type 2 diabetes mellitus accompanied with idiopathic hypoparathyroidism:One case report

对一例T2DM伴严重低钙血症患者行甲状旁腺素(PTH),降钙素,1,25二羟维生素D_3[1,25(OH)_2D_3],24 h尿钙、磷检测及头颅CT检查。其24 h尿磷偏低,血PTH和1,25(OH)_2D_3下降,头颅CT示小脑和基底节区的双侧多发对称性钙化,明确诊断为特发性甲状旁腺功能减退症,经补充钙剂、骨化三醇、钾镁盐及其他综合治疗,患者症状改善,代谢指标控制良好。

A type 2 diabetic patient accompanied with severe hypocalcemia received detections of serum parathormone（PTH）,calcitonin,1,25（OH）_2D_3,24 hours urine calcium and phosphorus and cerebral CT examination.24 h urine phosphorus,serum PTH and l,25（OH）_2D3 were all lowered.The cerebral CT examination showed bilateral multiple symmetric calcifications in the cerebellum and basal ganglia areas.So the patient was diagnosed as idiopathic hypoparathyroidism.After accepting supplement with calcium,calcitriol,potassium and magnesium salt as well as other related therapy,the patient recovered and obtained good metabolism control.
