喜炎平联合西医常规治疗对老年急性支气管炎患者肺功能和血气指标的影响

Effects of Xiyanping injection combined with conventional western medicine therapy on pulmonary function and blood gas analysis indexes in elderly patients with acute bronchitis

目的观察用喜炎平注射液联合西医常规治疗老年急性支气管炎患者的临床效果,探讨其对患者肺功能、血气分析指标的影响。方法选取120例老年急性支气管炎患者随机分为对照组和观察组,每组60例。对照组患者给予西医常规治疗,观察组患者在此基础上加用喜炎平注射液联合治疗,连续用药7 d。治疗后比较2组临床疗效,对用药安全性进行评估,评价患者治疗前后主要呼吸道中医证候积分,记录主要症状的改善时间,检测肺功能变化,进行血气分析比较,检测炎性因子水平变化。结果观察组临床有效率91.7%,明显高于对照组有效率76.7%,且观察组患者临床改善较对照组明显,其治疗后咳嗽、咳痰、发热、胸闷等中医证候评分显著低于对照组,且症状改善时间明显短于对照组,差异有统计学意义(P<0.05);观察组患者肺功能及血气分析指标改善优于对照组患者,FEV1、FVC、PEF%、Pa O2显著高于对照组,Pa CO2水平则显著低于对照组,差异有统计学意义(P<0.05);观察组患者炎性因子TNF-α、hs-CRP、IL-4水平显著低于对照组,差异有统计学意义(P<0.05)。结论喜炎平注射液治疗老年急性支气管炎患者疗效确切,利于加快呼吸道症状的缓解,提高肺功能,改善血气分析,其疗效机制与抗炎作用存在一定相关性。

Objective To observe the therapeutic effects of Xiyanping injection combined with conventional western medicine therapy on acute bronchitis in elderly patients,and to explore its effects on pulmonary function and blood gas analysis indexes of patients. Methods One hundred and twenty elderly patients with acute bronchitis were randomly divided into control group（ n = 60） and observation group（ n = 60）. The patients in control group were treated with routine western medicine,however,the patients in observation group,on the basis of control group,were treated with Xiyanping injection for 7days. After treatment,the therapeutic effects were observed and compared between two groups. The medication safety was evaluated,and the scores of TCM symptoms and syndromes were recorded before and after treatment,moreover,the improvement time of main symptoms,changes of pulmonary function,blood gas parameters and inflammatory factors levels weredetected for both groups. Results The clinical effective rate in observation group was 91. 7%,which was significantly higher than that（ 76. 7%） in control group（ P〈0. 05）. After treatment the scores of TCM symptoms and syndromes including cough,expectoration,fever,dyspnea in observation group were significantly lower than those in control group,and the improvement time of main symptoms in observation group was obviously shorter than that in control group（ P〈0. 05）. The improvement degree of lung function and blood gas analysis indexes in observation group were superior to those in control group,moreover,the levels of FEV1,FVC,PEF% and Pa O2 in observation group were significantly higher than those in control group,however,the levels of Pa CO2 in observation group were obviously lower than those in control group（ P〈0. 05）.Besides the levels of TNF-α,hs-CRP and IL-4 were significantly lower than those in control group（ P〈0. 05）. Conclusion Xiyanping injection is effective in treating acute bronchitis in elderly patients,which is helpful to relieve respiratory symptoms,improve pulmonary function and blood gas analysis indexes,and its action mechanism may be correlated to anti-inflammatory effects.
