老年妇女骨密度和髋部骨折的相关性分析 被引量:7

摘要 目的探讨与分析老年妇女骨密度和髋部骨折的相关性。方法选择2011年8月~2015年2月在深圳福田医院诊治的老年妇女髋部骨折患者72例作为骨折组,选择同期在该院进行体检的72例健康老年妇女作为对照组,两组均进行骨密度与骨代谢生化指标检测,同时进行骨质疏松情况的判断与相关性分析。结果骨折组与对照组沃氏三角区的骨密度分别为(0.76±0.12)g/cm2和(0.83±0.46)g/cm2,骨折组明显低于对照组(t=9.134,P=0.000)。骨折组的血清抗酒石酸酸性磷酸酶值比对照组低,骨钙素值比对照组高,其差异具有统计学意义(P〈0.05)。骨折组的骨质疏松与骨量减少发生率都明显高于对照组(P〈0.05)。Pearson相关分析显示髋部骨折的发生与骨质疏松情况、骨钙素含量呈现明显正相关性(r=0.344,r=0.225,P〈0.05),而与骨密度、抗酒石酸酸性磷酸酶含量呈现明显负相关性(r=-0.246,r=-0.341,P〈0.05)。结论老年妇女髋部骨折多伴随有骨密度下降与骨质疏松情况,同时骨代谢生化指标表达紊乱,髋部骨密度可作为预测和评价老年妇女髋部骨折的主要检测指标。 Objective To discuss the correlation between mineral density and hip fractures in elderly women. Methods From Aug. 2011 to Feb. 2015,72 elderly women with hip fractures in our hospital were selected as the fracture group,and 72 healthy older women were selected as the control group. The bone density and biochemical parameters of bone metabolism were detected in the two groups. Simultaneously,osteoporosis was determined and the correlation analysis was conducted. Results The Ward's triangle bone density in the fracture group and the control group were( 0. 76 ± 0. 12) g / cm2and( 0. 83 ± 0. 46) g / cm2,which was significantly lower in the fracture group than the control group( P 0. 05). The serum tartrate-resistant acid phosphatase value of the fracture group was significantly lower than that of the control group,while the osteocalcin value was significantly higher than the control group,with significant differences( P 0. 05). The incidence rates of osteoporosis and osteopenia in the fracture group were significantly higher than the control group( P 0. 05). Pearson correlation analysis showed that the incidence of hip fractures was significantly positively correlated to the osteoporosis and the osteocalcin contents( r = 0. 344,r = 0. 225,P 0. 05),and were significantly negatively correlated to the bone density and tartrate-resistant acid phosphatase contents( r =- 0. 246,r =- 0. 341,P 0. 05). Conclusion The elderly women with hip fractures are more accompanied by decreased bone mineral density and osteoporosis. Meanwhile,the expressions of bone metabolism biochemical indicators are disordered. Hip bone density can be used as the main test indicator to predict and evaluate hip fractures in elderly women.
