肠外置术在超低位直肠癌前切除术中的应用意义

Application significance of intestinal exteriorization in the anterior resection for ultra-low rectal carcinoma

目的了解肠外置术在超低位直肠癌前切除术中的应用意义。方法将98例超低位直肠癌患者随机分为对照组和试验组,每组49例。对照组采用直肠癌超低位前切除术,而试验组在对照组基础上加用肠外置术。比较两组在吻合口瘘、排气时间、住院时间及生活质量方面的差异。结果试验组吻合口瘘发生率显著低于对照组(2.0%vs 20.4%,P<0.01);排气时间、住院时间均显著短于对照组[分别为(36.6±4.9)h vs(44.8±5.2)h,(21.3±3.6)d vs(28.6±4.7)d,均P<0.01),生活质量评分显著高于对照组[(73.2±8.5)vs(56.9±7.6),P<0.01]。结论肠外置术对超低位直肠癌前切除术效果显著,其吻合口瘘发生率少,患者住院时间短、恢复快,生活质量高。

Objective To understand the application significance of intestinal exteriorization in the anterior resection for ultra-low rectal carcinoma. Methods 98 patients with ultra-low rectal carcinoma were randomly divided into Control Group and Test Group with 49 cases in each group. Control Group was subject to anterior resection for ultra-low rectal carcinoma while Test Group was subject to intestinal exteriorization on the basis. The two groups were compared in terms of anastomotic fistula, exhaust time, hospital stay and quality of life. Results The incidence of anastomotic fistula in Test Group（2.0%） was significantly lower than that in Control Group（20.4%）（P〈0.01）; the exhaust time and hospital stay in Test Group were significantly shorter than those in Control Group（P〈0.01）; the quality of life in Test Group was significantly higher than that in Control Group（P〈0.01）. Conclusion The intestinal exteriorization has an significant effect on anterior resection for ultra-low rectal carcinoma. It can reduce the incidence of anastomotic fistula, shorten the hospital stay, promote the patient＇s recovery, and improve the patients＇ quality of life.
