血清同型半胱氨酸与冠脉病变支数的相关研究

The Cottelation Between Blasma Homocysteine and Lesion Severity In Coronary Artery Disease(CAD)Patients

目的研究入选患者同型半胱氨酸水平(HCY)与冠状动脉粥样硬化性心脏病(CAD)及其血管病变支数的关系。方法 200例患者按冠脉造影结果分为冠状动脉病变(CAS)组(155例)和冠状动脉正常(CAN)组(45例),根据冠状动脉病变程度进一步将冠脉病变组分为:单支病变组(70例),双支病变组(54例),3支病变组(31例)。用高效液相色谱法测定血浆Hcy水平,研究其与冠心病的相关性及与病变严重程度的关系。结果冠脉病变组血清HCY水平显著高于冠脉正常组(P=0.01),血清HCY水平在冠脉无病变、单支病变、双支病变、三支病变组中呈逐级增高趋势,数值为(7.78±2.88)μmol/L、(14.32±4.56)μmol/L、(15.45±3.74)μmol/L、(18.21±5.23)μmol/L,多重比较结果提示:3支病变组与无病变组有高度显著性差异(P<0.01),双支、单支病变组与无病变组有显著性差异(P<0.05),单支与双支病变组、3支病变组,双支与3支病变组比较无显著性差异(P>0.05)。结论血浆HCY水平与冠状动脉病变支数呈正相关。

Objective Through measuring the plasma level of Homocysteine（Hcy）in CAD patients with difference lesion severity,we sought to assess whether plasma Hcy level can be independently associated with lesion severity.Mathods Patients undergoning coronary angiography were divided into four subgroups：Coronary single vessel lesion（n=70）,Double coronary artery lesions（n=54）,Three coronary artery lesions（n=31）and the control patients（n=45）,clinical and biochemical characteristics were collected,the plasma level of Hcy be measured.Results（1）Plasma level of Hcy in patients with CAD was higher than those in the control group.Moreover,plasma Hcy level in subgroup with CAD were：Coronary single vessel lesion（14.32±4.56）umol/L、Double coronary artery lesions（15.45±3.74）umol/L、Three coronary artery lesions（18.21±5.23）umol/L,the contrast between them is significance（P〈0.05）.Conclusion（1）Plasma level of Hcy in patients with CAD was higher than those in the control group,Hcy level was positively correlated with the lesion severity of CAD,the results suggests that Hcy maybe a bad adipocytokin in CAD,it can be used as a index of indicate in CAD.
