不同固定方式治疗老年胸腰椎爆裂型骨折对比分析

目的探讨不同固定方式治疗老年胸腰椎爆裂性骨折的临床疗效。方法本研究选择的对象为2011年6月至2015年5月我院收治的98例老年胸腰椎爆裂患者,根据患者在复位固定手术时所采取的固定方式的不同将其分为两组,对照组患者行跨伤椎弓根复位固定方式;观察组患者手术中行经伤椎弓根复位固定。观察的指标有两组患者围手术期常规指标以及两组患者治疗过程中的不良反应发生率。结果观察组患者的手术时间、术中出血、住院时间以及X射线透视时间均显著性的低于对照组患者,两组患者进行比较有统计学意义(P<0.05);对照组患者神经损伤、螺钉松动、螺钉断裂以及椎间盘退变等常见并发症的发生率为20.4%,观察组不良反应发生率为6.1%,两组患者进行比较有统计学意义(P<0.05)。结论经伤椎椎弓根螺钉复位固定手术治疗老年胸腰椎骨折治疗效果良好,不良反应发生率较少,是治疗临床老年胸腰椎爆裂一种较为理想的治疗方案,值得对该治疗方案进行进一步深入研究。

Objective To investigate the effects of different fixation treatment of senile thoracolumbar burst fractures.Methods The objects chosen for this study were 98 cases of senile thoracolumbar burst fracture from June 2013 to May2015,according to different fixation methods they were divided into two groups.The control group used fixation method across the injured pedicle;observation group underwent pedicle fixation via the injured pedicle.Observation indicators were perioperative conventional indicators and the incidence of adverse reactions in the course of treatment.Results The patient＇s operation time,blood loss,hospital stay,and X-ray fluoroscopy time were significantly lower than those of control group,the differences were statistically significant（P〈0.05）;the incidence of complications including nerve injury patients,screw loosening,screw breakage and disc degeneration etc.in the control group was 20.4%,and that in observation group was 6.1%,the difference was statistically significant（P〈0.05）.Conclusion The pedicle screw fixation surgical treatment via the injured pedicle has a good effect and less incidence of adverse reactions,it＇s an ideal treatment for senile thoracolumbar fracture and worth further in-depth study.
