滋阴健脾祛瘀汤联合奥沙利铂、替吉奥对晚期胃癌患者生活质量及免疫学指标的影响

Effects of Ziyin-jianpi-quyu formula combined with oxaliplation and S-1 capsule on life quality and immunological indicators in patients with advanced gastric cancer

目的观察比较滋阴健脾祛瘀汤联合奥沙利铂、替吉奥对晚期胃癌患者生活质量及免疫学指标的影响。方法将86例晚期胃癌患者随机分为2组,对照组43例采用奥沙利铂+替吉奥联合化疗,治疗组43例在对照组基础上加滋阴健脾祛瘀汤。2组均3周为1个疗程,治疗4个疗程后统计疗效;采用欧洲癌症研究与治疗组织(EORTC)生命质量测定量表QLQ-C30评估2组患者生活质量;检测治疗前、后T淋巴细胞亚群CD3^+、CD4^+、CD8^+变化情况;参照世界卫生组织(WHO)抗癌药物常见毒副反应分级标准比较2组Ⅲ度及以上毒副反应发生率。结果治疗组有效率44.19%,对照组有效率30.23%,2组近期实体瘤有效率比较差异无统计学意义(P>0.05),疗效相当。2组治疗后QLQ-C30量表社会功能及总体健康评分均较本组治疗前提高,恶心呕吐、疲乏、疼痛、食欲丧失评分降低,比较差异均有统计学意义(P<0.05);治疗组治疗后躯体功能、角色功能、社会功能、总体健康评分及CD4^+、CD3^+水平高于对照组(P<0.05),恶心呕吐、疲乏、疼痛、食欲丧失、便秘评分及Ⅲ度及以上骨髓抑制、恶心呕吐、白细胞计数减少及肝功能损害发生率低于对照组,比较差异均有统计学意义(P<0.05)。结论滋阴健脾祛瘀方联合奥沙利铂、替吉奥治疗可提高晚期胃癌患者生活质量及免疫功能,疗效确切。

Objective To observe the effects of Ziyin- jianpi- quyu formula combined with oxaliplation and S- 1 capsule on life quality and immunological indicators in patients with advanced gastric cancer. Methods86 subjects with advanced gastric cancer were randomly divided into two groups. 43 subjects in control group were treated by combined chemotherapy of oxaliplation and S- 1 capsule. 43 subjects in treatment group were treated by f Ziyin- jianpi- quyu formula on the basis of control group treatment. The course was three weeks in two groups. The curative effect was evaluated after four courses. The life quality was evaluated by the european qrganization for research and treatment of cancer quality of life questionnaire- stomach cancer（ EORTC,QLQ- C30）. The changes of CD3＋,CD4＋and CD8＋before and after treatment were detected in two groups. The toxic and side effects of level-Ⅲ and above were recorded which according to WHO anticancer drug toxicity grading standards,and the occurrence rate was evaluated. Results The effective rate in treatment group（ 44. 19%） was superior to that in control group（ 30. 23%,P〈0. 05）. There was no statistical difference on short- term efficacy of solid tumors between two groups（ P〈0. 05）. The social function and general health score after treatment were increased in two groups,and the scores of nausea,vomiting,fatigue,pain and appetite loss after treatment were decreased in two groups,with statistical differences（ P〈0. 05）. The scores of constitutional function,role function,social function and general health and the levels of CD4＋,CD3＋after treatment in treatment group were higher than those in control group（ P〈0. 05）.The scores of nausea,vomiting,fatigue,pain,appetite loss and constipation after treatment in treatment group was lower than those in control group,as well as the occurrence rate of myelosuppression,nausea, vomiting, leucopenia and live function damage with level- Ⅲ or above（ P〈0. 05）. Conclusion Ziyin- jianpi- quyu formula combined with oxaliplation and S- 1 capsule can enhance the life quality and immunological indicators in patients with advanced gastric cancer,has exact effects.
