预见性护理在中西医结合治疗慢性硬膜下血肿中的应用

Application of predictive nursing in Integrated Chinese and Western medicine treatment of chronic subdural hematoma

目的观察预见性护理在中西医结合治疗慢性硬膜下血肿(CSDH)中的应用。方法将80例CSDH患者随机分为2组,均行钻孔引流术,术后给予参附注射液治疗,常规护理组40例实施常规护理,预见性护理组40例给予预见性护理。比较2组疗效,观察中国卒中量表(CSS)评分和Barthel指数(BI)评分变化,比较2组引流管拔除时间、住院时间和并发症发生率。结果预见性护理组总有效率97.50%,常规护理组95.00%,2组总有效率比较差异无统计学意义(P>0.05),2组疗效相当。2组护理后均较本组护理前CSS评分降低(P<0.05),BI评分提高(P<0.05)。预见性护理组护理后较常规护理组CSS评分降低、BI评分提高更明显(P<0.05)。预见性护理组引流管拔除时间、住院时间均少于常规护理组(P<0.05)。预见性护理组并发症发生率50.00%,常规护理组22.50%,2组并发症发生率比较差异有统计学意义(P<0.05),预见性护理组低于常规护理组。结论预见性护理能有效提高中西医结合治疗CSDH的效果,缩短治疗时间,降低术后并发症发生率。

Objective To observe the application of predictive nursing in Integrated Chinese and Western medicine treatment of chronic subdural hematoma（ CSDH）. Methods 80 CSDH patients were randomly divided into two groups,received Burr- hole irrigation and drainage under general anesthesia,and then were treated by Shenfu injection. 40 patients in routine nursing group were received routine nursing,and 40 patients in predictive nursing group were received predictive nursing. The curative effect was compared between two groups. The scores of Chinese Stroke Scale（ CSS） and Barthel index（ BI） before and after treatment were observed. The drainage tube removal time,hospitalization time and the occurrence rate of postoperative complications were compared. Results The total effective rate in predictive nursing and routine nursing group was 97. 50% and 95. 00%,respectively,with no statistical differences（ P〈0. 05）. The CSS score after nursing was decreased in two groups（ P〈0. 05）,and BI was increased（ P〈0. 05）. The improvements of CSS and BI in predictive nursing group were more significant than those in routine nursing group（ P〈0. 05）. The drainage tube removal time and hospitalization time in predictive nursing group were lower than those in routine nursing group（ P〈0. 05）. The occurrence rate of postoperative complications in predictive nursing group（ 5. 00%） was lower than that in routine nursing group（ 22. 50%,P〈0. 05）. Conclusion Predictive nursing can effectively enhance the therapeutic effect in Integrated Chinese and Western medicine treatment of CSDH,can shorten treatment times,and reduce postoperative complications.
