中药大补元煎合优甲乐治疗脾肾阳虚型桥本甲状腺炎甲状腺功能减低30例临床观察

Clinical effect of Dabuyuanjian Decoction combining Euthyrox Tablet in treating hypothyrea of 30 cases with hashimoto's hypothyroidism due to deficiency of both spleen and kidney

目的观察中药大补元煎合优甲乐治疗脾肾阳虚型桥本甲状腺炎甲状腺功能减低的临床疗效。方法选取2012年1月至2015年8月在我院住院的脾肾阳虚型桥本甲状腺炎甲状腺功能减低患儿60例,随机分为对照组和治疗组,各30例。对照组给予口服优甲乐片,治疗组在对照组治疗方法的基础上给予中药大补元煎口服。2组均连续治疗2月,比较2组患儿的临床疗效和甲状腺激素水平及恢复时间的变化情况。结果对照组总有效率为73.33%,治疗组为90.00%,2组比较差异有统计意义(P<0.05)。与同组治疗前比较,治疗后2组甲状腺游离三碘甲腺原氨酸(FT_3)、游离甲状腺素(FT_4)均显著升高(P<0.05或P<0.01),促甲状腺激素(TSH)、抗甲状腺球蛋白抗体(TGAb)、抗甲状腺过氧化物酶抗体(TPOAb)均显著降低(P<0.05或P<0.01);治疗后,治疗组FT_3,FT_4,TSH,TGAb,TPOAb与对照组比较,差异有统计意义(P<0.05或P<0.01)。治疗后治疗组患儿FT_3,FT_4,TSH激素水平恢复时间短于对照组,差异有统计意义(P<0.05)。结论中药大补元煎能提高脾肾阳虚型桥本甲状腺炎甲状腺功能减低患儿的临床疗效,升高甲状腺激素水平,缩短激素水平恢复时间,对免疫调节有重要作用。

Objective To observe the clinical effect of Dabuyuajian Decoction combining Euthyrox Tablet in treating hypothyrea of 30 cases with hashimoto ＇s hypothyroidism due to deficiency of both spleen and kidney.Methods Totally 60 children suffering from hashimoto＇s thyroditis were randomized into control group and treatment group,30 cases in each group. The control group took Euthyrox Tablet orally,based on that,the treatment group was intervened with the decoction of Dabuyuanjian additionally,the treatment lasted 2 months. Clinical effects,recovery time of thyroid hormone were compared between 2 groups after treatment. Results The total effective rate of control group was 73. 33% and that of treatment group was 90. 00%,there was a significant difference（ P〈0. 05）. After treatment,FT3,FT4 in two groups increased obviously（ P〈0. 05 or P〈0. 01）,TSH,TGAb,TPOAb decreased significantly（ P〈0. 05 or P〈0. 01）. Difference also existed in comparing FT3,FT4,TSH,TGAb and TPOAb between two groups（ P〈0. 05 or P〈0. 01）. Recovery time of thyroid hormone in treatment group was much less than in control group,the difference was significant（ P〈0. 05）. Conclusion The therapy has a satisfactory effect in treating dysfunctional hashimoto＇s thyroditis,also plays an important role in immunoregulation.
