车祸伤合并慢性呼吸衰竭导致肺炎的护理临床观察

Clinical Observation and Nursing of Pneumonia Caused by a Traffic Injury Complicated with Chronic Respiratory Failure

目的探讨车祸伤合并慢性呼吸衰竭的临床护理方法。方法选取车祸伤合并呼吸衰竭的患者,按护理方式不同将患者随机分为观察组和对照组,各16例。对照组给予常规护理,观察组在常规护理的基础上实施优质护理并给予人血静脉丙种蛋白辅助治疗的工作方案。对比两组患者的治疗效果,对疼痛指标,呼吸机使用时间,住院时间等指标进行比较及对护理服务的满意度进行测评,采用Zung焦虑自评量表(SAS)和抑郁自评量表(SDS)对患者护理前后心理状态进行评分。结果手术前,满意度无显著性差异(P>0.05),手术后观察组患者SAS及SDS评分改善明显优于对照组(P<0.05),疼痛感观察组较对照组也有明显好转。结论优质护理工作有助于提高患者的护理服务满意度,促进患者康复,值得临床推广。

Objective To investigate the clinical nursing of patients＇ traffic injury with chronic respiratory failure. Methods ICU Patients with respiratory failure due to traffic accidents in our hospital were randomly chosen from April 2012 to August 2014, and then according to different nursing methods, patients were randomly divided into observation group and control group, each with 16 cases. Patients in the control group were given routine nursing care, while patients in observation group on the basis of routine nursing care were given high quality nursing care together with the adjuvant therapy of Intravenous gamma globulin in our hospital. The curative effects, the pain index, mechanical ventilation time, hospitalization time were compared and the satis- faction degree of nursing service was evaluated, and using the Zung Self-rating Anxiety Scale （SAS） and Self-rating Depression Scale （SDS） to assess the psychological state of these patients before and after nursing care service. Results Before the operation, there was no significant difference in satisfaction degree （P〉 0.05）; after the operation, their SAS and SDS scores in observation group were improved significantly, which is better than the control group （P 〈 0.05）, the pain index in observation group had ob- vious improvement than that in the control group. Conclusion High quality nursing care helps to improve patients＇ satisfaction with nursing service as well as patients＇ rehabilitation, which is worthy of clinical promotion.
