多层螺旋CT在心房颤动患者射频消融术前的应用价值研究 被引量:8

摘要 目的探讨多层螺旋CT对心房颤动(房颤)患者射频消融术前肺静脉评价情况的应用价值。方法收集我院拟行射频消融术的房颤患者35例为房颤组,男25例,女10例,另选进行冠状动脉体检的35例正常者为对照组,男22例,女13例,分析入选者的CT资料。结果两侧肺静脉的上下径均较前后径粗,两侧上肺静脉的上下径及前后径均较两侧下肺静脉粗,各肺静脉的上下径均较前后径粗(P<0.05);各肺静脉两两比较有显著差异(P<0.05,P<0.01)。房颤组左、右上侧肺静脉前后径[(1.81±0.29)cmvs(1.49±0.24)cm,P=0.001,(1.75±0.34)cm vs(1.56±0.23)cm,P=0.002]、上下径[(2.27±0.28)cmvs(1.85±0.24)cm,P=0.001,(2.01±0.41)cmvs(1.79±0.27)cm,P=0.002]均较对照组增大,2组两侧下肺静脉上下径有显著差异(P<0.05)。两上肺静脉开口走行较两下肺静脉开口角度更大(P<0.05)。结论多层螺旋CT在射频消融术前指导定位具有重要价值。 Objective To study the value of multilayer spiral CT in assessing pulmonary veins of AF patients before radiofrequency catheter ablation.Methods Thirty-five AF patients admitted to our hospital for radiofrequency catheter ablation served as an AF group and 35 healthy persons undergoing coronary angiography served as a control group in this study.Their CT data were analyzed.Results The upper and lower diameters of bilateral pulmonary veins were longer than the anterior and posterior diameters of bilateral pulmonary veins,the upper and lower diameters of bilateral superior pulmonary veins were longer than the anterior and posterior diameters of bilateral inferior pulmonary veins in 2groups(P〈0.05,P〈0.01).The anterior and posterior diameters of left and right superior pulmonary veins ostia were longer in AF group than in control group(1.81±0.29 cmvs 1.49±0.24 cm,P=0.001,1.75±0.34 cmvs 1.56±0.23 cm,P=0.002)and the upper and lower diameters of bilateral inferior pulmonary veins ostia were longer in AF groupp than in control group(P〈0.05).The opening angle of bilateral superior pulmonary veins was larger than that of bilateral inferior pulmonary veins(P〈0.05).Conclusion Multilayer spiral CT plays an important role in locating the pulmonary veins before radiofrequency catheter ablation.
