氟西汀和舍曲林、米氮平对抑郁障碍患者糖代谢的影响

摘要 目的探讨氟西汀和舍曲林、米氮平对抑郁障碍患者糖代谢的影响。方法将96例抑郁障碍患者采用随机数字表法随机分为米氮平组、氟西汀组、舍曲林组,每组32例;治疗观察期均为12周;3组患者于治疗前及治疗后第4、12周末测定血浆神经肽Y和瘦素水平,同时检测空腹血糖、胰岛素(INS)水平进行比较。结果 3组在第4周末与基数比较末出现空腹血糖过高,差异无统计学意义(P均>0.05),而在第12周末,米氮平组空腹血糖过高,与基数比较差异有统计学意义(P均<0.05)。3组治疗前及治疗后胰岛素、NPY及瘦素与基数比较差异无统计学意义(P均>0.05)。结论米氮平治疗既往无糖代谢异常的抑郁障碍患者可出现空腹血糖增高。 OBJECTIVE To investigate the effect of fluoxetine and Seculin,mirtazapine on glucose metabolism in patients with depressive disorder. METHODS 96 patients were randomly divided into three groups with 32 in each,who were treated with mirtazapine,fluoxetine,or sertraline for 12 weeks. The FBG,fasting insuline and plasma NPY,leptin levels were measured at baseline and at weeks 4 and 12. RESULTS The 3 group in the fourth week and the base was not impaired fasting blood glucose,the difference was not statistically significant( p〈0. 05). And at the end of the twelfth week,mirtazapine group fasting blood glucose was high and there was significant difference between the base( p〈0. 05). The three groups had no significant difference before and after the treatment comparison of insulin,NPY and leptin with baseline( p〈0. 05). CONCLUSION Mirtazapine in the treatment of depression in patients with no previous history of abnormal glucose metabolism may be impaired fasting blood glucose.
