妊娠期脐带异常对新生儿脐血神经元特异性烯醇化酶的影响及临床意义 被引量:4

The effect of gestational umbilical cord abnormality on neuron specific enolase of neonate cord blood and the clinical significance

摘要 目的观察不同类型脐带异常新生儿脐血中神经元特异性烯醇化酶(Neuron Specific Enolase,NSE)水平变化,总结新生儿脑损伤诊治的相关因素。方法选择2013年5月至2014年12月在咸阳市中心医院产科分娩的妊娠期脐带异常新生儿80例为观察组,同期正常出生的新生儿50例为对照组,采用ELISA法检测新生儿脐血NSE水平并进行分析比较。结果观察组脐血NSE含量较对照组明显升高(P<0.05);脐带绕颈组中NSE值与绕颈周数呈正比(P<0.05);脐带打结组中真打结组NSE值高于假打结组(P<0.05);脐带扭转组中NSE值随扭转周数增多而增高(P<0.05)。结论脐带异常可引起脐血中NSE值升高,而脐血NSE水平升高与脑缺氧缺血损伤密切相关,脐血NSE可能做为预测新生儿宫内脑损伤的指标。 Objective Observe the changes of neuron specific enolase (NSE) in neonate cord blood in cases with different types of abnormal umbilical cord, and summarize the related factors of diagnosis and treatment of neonatal brain injury. Methods 80 neonates with gestational umbilical cord complications born in Xianyang Central Hospital from May 2013 to December 2014 were selected as study group, and 50 neonates with grossly normal umbilical cord were selected as control group. ELISA method was used to detect the level of NSE in serum of umbilical cord. Results The level of NSE of study group was significantly higher than that of control group ( P 〈 0. 05 ) ; the level of NSE of cord around neck subgroup was directly proportional to the circle numbers of cord around neck ( P 〈 0. 05 ) ; the level of NSE of subgroup of real umbilical cord knots was higher than that of false knots subgroup ( P 〈 0. 05 ) ; the level of NSE of subgroup of umbilical cords twists increased when twists circles increase ( P 〈 0. 05 ). Conclusion Umbilical cord complications can cause the increase of NSE, and the level of NSE is closely correlated with brain hypoxia, so it could be a marker to indicate the intrauterine brain injury.
