妊娠期生殖道人乳头瘤病毒感染对妊娠结局和新生儿的影响

Impact of Human Papilloma Virus(HPV) Infection in the Genital Tract on Pregnancy Outcomes and Newborn

目的:探讨妊娠期妇女合并生殖道人乳头瘤病毒(HPV)感染对妊娠结局和新生儿的影响,总结其临床疗效及治疗体会。方法:选择本院2014年1-9月妇产科收治的55例妊娠合并生殖道HPV感染患者为研究组,对其临床病历资料进行回顾性分析,并以同期53例未感染HPV的妊娠妇女为正常对照组。结果:妊娠期妇女HPV感染率为39.3%(55/140),其中30.9%(17/55)为临床型感染,40.0%(22/55)亚临床型感染,29.1%(16/55)潜伏型感染。两组的分娩情况、不良妊娠结局比较差异均无统计学意义(P>0.05)。研究组新生儿HPV感染11例(23.4%),不同分娩方式的羊水、脐血、胎盘及咽喉分泌物的HPV阳性率比较差异均无统计学意义(P>0.05)。结论:妊娠合并下生殖道HPV感染可经母婴垂直传播给新生儿,但对妊娠结局影响不大,不同分娩方式对新生儿HPV阳性率无显著影响,高危型HPV感染是浸润性宫颈癌的危险因素,妊娠期妇女应加强早期筛查并治疗。

Objective：To investigate the impact of HPV infection in genital tract of pregnant women on pregnancy outcomes and newborn,summary its clinical efficacy and experience.Method：55 cases with HPV infected in obstetrics and gynecology department of our hospital were selected from January to September 2014 as research group,their clinical data were retrospectively analyzed,other 53 cases of non HPV infected were selected as control group.Result：The infection rate of pregnant women was 39.3%（55/140）,including 30.9%（17/55）of clinical infection,40%（22/55） of sub clinical infection and 29.1%（16/55） latent infection.There was no significant difference between the two groups in the delivery condition and adverse pregnancy outcome（P0.05）.The infection of HPV in newborn were 11 cases（23.4%）,the positive rate of HPV in amniotic fluid,umbilical cord blood,placenta and throat secretions of newborn with different delivery modes had no satistically significance（P0.05）.Conclusion：HPV can be transmitted vertically from mother to newborn,but pregnancy combined with HPV infection has no obvious effect on pregnancy outcomes.Different delivery modes has no significant effect on HPV positive rate of newborn.High risk HPV typing is a risk factor for invasive cervical cancer,early screening and symptomatic treatment is necessary in pregnant women.
