经直肠双重超声造影在直肠癌术前T分期中的价值

Diagnostic value of double contrast-enhanced transrectal ultrasound in preoperative T staging of rectal carcinoma

目的探讨经直肠双重超声造影在直肠癌术前T分期和定位的应用价值。方法 38例直肠癌患者术前行经直肠双重超声造影和常规经腹超声检查,对直肠肿瘤定位、血流情况、肠壁浸润深度及微循环灌注模式等声像表现进行评价,并与术后病理结果进行对比。结果经直肠双重超声造影对直肠肿瘤血流分级诊断与常规经腹超声比较,差异有统计学意义(P<0.05);经直肠双重造影的T分期与定位诊断的准确率分别为78.9%和92.1%。结论经直肠双重超声造影是评价直肠癌术前T分期和定位的有效方法。

Objective To evaluate the value of preoperative T staging and location in diagnosis of rectal carcinoma with double contrast-enhanced transrectal ultrasound（DCUS）.Methods Thirty-eight patients with rectal carcinoma were examined by DCUS and transabdominal ultrasound preoperatively. The position,blood flow,invasion depth,perfusion patterns of the tumor were observed and the staging was compared with final pathologic results after surgery.Results There was significant difference in the diagnosis of the degree of blood flow in rectal carcinoma between DCUS and transabdominal ultrasound（P〈0.05）. The accuracy of DCUS in determining the T stage of rectal carcinoma and location diagnosis were 78.9% and 92.1% ,respectively. Conclusion DCUS is an effective tool for preoperative T staging and location diagnosis of rectal carcinoma.
