PVP和PKP治疗骨质疏松性椎体压缩骨折的疗效比较 被引量:10

摘要 目的:探讨椎体成形术(PVP)和椎体后凸成形术(PKP)治疗单节段骨质疏松性椎体压缩骨折(OVCF)的疗效。方法:2013年1月至2014年1月应用PVP或PKP共治疗了83例单节段OVCF患者。其中PVP治疗组40例40节椎体,PKP治疗组43例43节椎体。比较两组的手术时间、出血量及骨水泥注入量。采用疼痛视觉模拟评分(VAS)、伤椎前中部高度及后凸畸形Cobb角等指标来评估两组疗效。结果:两组出血量和骨水泥注入量比较,差异无统计学意义(P>0.05)。两组手术时间比较,PVP组短于PKP组,差异有统计学意义(P<0.05)。两组患者术后VAS评分与术前相比差异有统计学意义(P<0.05),而两组间比较差异无统计学意义(P>0.05)。两组患者术后椎体前、中部高度,术后后凸角与术前相比,差异有统计学意义(P<0.05),且PKP组在椎体前中部高度的恢复,后凸畸形Cobb角的矫正方面优于PVP组。PVP组14个椎体(35%)、PKP组8个椎体(18%)出现骨水泥渗漏,但两组骨水泥渗漏率相比差异无统计学意义(P>0.05)。结论:PVP和PKP均能有效缓解疼痛,且PKP在椎体前中部高度的恢复及后凸畸形的矫正方面优于PVP,但两种治疗方法在减轻患者疼痛方面并无明显差异性。
