泮托拉唑与阿莫西林、呋喃唑酮、胃铋镁联合治疗胃溃疡的疗效观察

Observation on Efficacy of Pantoprazole Combined with Amoxicillin, Furazolidone and Compound Bismuth and Magnesium in Treatment of Gastric Ulcer

目的:探讨泮托拉唑与阿莫西林、呋喃唑酮、胃铋镁联合治疗胃溃疡的疗效。方法:选取云南省第二人民医院2014年10月—2015年9月收治的消化胃溃疡患者160例,按随机数字表法分为2组各80例。观察组患者给予泮托拉唑与阿莫西林、呋喃唑酮、胃铋镁联合治疗(四联疗法),对照组患者给予枸橼酸铋钾片与替硝唑片、克拉霉素片联合治疗(胃三联法)。治疗2周后,对2组患者的疗效、幽门螺杆菌转阴率、平均中位溃疡愈合时间及不良反应发生率进行比较。结果:观察组患者总有效率为91.25%(73/80),对照组为76.25%(61/80),差异有统计学意义(χ2=6.613,P<0.05);观察组患者幽门螺杆菌转阴率为86.25%(69/80),对照组为66.25%(53/80),差异有统计学意义(χ2=8.835,P<0.05);观察组患者不良反应发生率为7.50%(6/80),对照组为13.75%(11/80),差异有统计学意义(χ2=1.645,P<0.05)。结论:四联疗法治疗胃溃疡具有疗效显著、不良反应少、幽门螺杆菌转阴率高等优点,值得临床推广。

OBJECTIVE： To probe into the efficacy of pantoprazole combined with amoxicillin,furazolidone and compound bismuth and magnesium in treatment of gastric ulcer. METHODS： 160 cases of gastric ulcer admitted into the Second People＇s Hospital of Yunnan from Oct. 2014 to Sept. 2015 were selected to be divided into two groups via the random number table,with 80 cases in each. The observation group were treated with pantoprazole combined with amoxicillin,furazolidone and compound bismuth and magnesium（ quadri-combination therapy）; while the control group received Bismuth potassium citrate tablets combined with Tinidazole tablets,Clarithromycin tablets（ stomach triple therapy）; after treatment of two weeks,the efficacy,negative conversion ratio of helicobacter pylori,healing time of the average median ulcers and incidence of adverse drug reactions were compared between two group of patients. RESULTS： The total effective rate of observation group was 91. 25%（ 73 /80）,and control group was76. 25%（ 61 /80）,the difference was statistically significant（ χ2= 6. 613,P 0. 05）. The negative conversion ratio of helicobacter pylori in observation group was 86. 25%（ 69 /80）,and in control group was 66. 25%（ 53 /80）,the difference was statistically significant（ χ2= 8. 835,P 0. 05）. The incidence of adverse drug reactions of observation group was 7. 50%（ 6 /80）,and control group was 13. 75%（ 11 /80）,with statistical significance（ χ2= 1. 645,P 0. 05）. CONCLUSIONS： The efficacy of pantoprazole combined with amoxicillin,furazolidone and compound bismuth and magnesium in treatment of gastric ulcer is significant,with fewer adverse drug reactions and higher negative conversion ratio of helicobacter pylori. It is worthy of clinical application and promotion.
