精神科病区退药原因分析及对策探讨

Analysis and Discussion on Causes of Drug Repercussion and Countermeasures

目的:了解厦门市仙岳医院(以下简称"我院")病区的退药情况。方法:收集并整理我院2014年7月1日—2015年6月30日计算机系统中的退药记录,对其退药原因进行分类统计,并提出应对措施。结果与结论:我院各病区均存在退药情况,以抗精神病药(占27.79%)、注射剂(占31.47%)退药为主,抗焦虑抑郁药、心境稳定剂、镇静催眠药的退药情况较少,分别占6.87%,15.46%、8.40%。退药原因主要包括更改医嘱、药品品规选择不合适、计算机录入错误、药品库存原因及患者出现不良反应等。病区频繁退药,不仅增加了医务人员的工作量、影响了药品的质量管理,还会增加医患矛盾。建议进一步加强医患沟通,建立并完善各种规章制度,强化医护人员的责任意识,以有效减少退药现象的发生。

OBJECTIVE： To investigate the drug repercussion in Xiamen Xuanyue Hospital（ hereinafter referred to as＂our hospital＂）. METHODS： Records of drug repercussion from from Jul. 1st 2014 to Jun. 30 th 2015 were collected and analyzed statistically, and correspond countermeasures were put forward. RESULTS CONCLUSIONS： Drug repercussion occurred in all wards of our hospital,and antipsychotic drugs（ 27. 79%） and injections（ 31. 47%） took the lead,while the ratio of repercussion of antianxiety and antidepressants,mood stabilizers and sedative hypnotics were relatively low,respectively 6. 87%,15. 46% and 8. 40%. The causes of drug repercussion were mainly adjustment of medical orders,improper selection of drug specifications,faults of computer-operation,shortage of drug stock and adverse drug reactions（ ADRs）,etc. All-too-frequent drug repercussion in all wards would increase the workload of medical staffs,affect the quality management of drugs,and increase the conflicts of doctors and patients. It is necessary to further strengthen the communication between doctors and patients, establish and improve various rules and regulations,strengthen the responsibility of medical staffs,so as to reduce the incidence of drug repercussion effectively.
