慢性心力衰竭伴贫血患者红细胞分布宽度水平的研究 被引量:8

The relationship between red blood cell distribution width and the chronic heart failure with anemia

摘要 目的 探讨红细胞分布宽度(RDW)与慢性心力衰竭伴贫血之间的关系。方法 采集2014年7~12月本院内科住院患者125例,其中单纯慢性心力衰竭无贫血患者63例,慢性心力衰竭伴贫血患者62例,对照组为正常体检人群60例。采用全自动血液分析仪测定RDW值。结果 慢性心衰伴贫血组患者RDW水平明显高于单纯慢性心衰、对照组[(15.44±1.05)%〉(14.46±0.75)%〉(12.38±0.58)%,均P〈0.01]。慢性心衰伴贫血组RDW与其他检测指标的相关性显示:RDW与NT-pro BNP、尿酸、hs-CRP、血肌酐正相关(r=0.78,P〈0.01;r=0.51,P〈0.01;r=0.42,P〈0.01;r=0.35,P〈0.01),与血红蛋白负相关(r=-0.72,P〈0.01)。单纯慢性心衰组相关指标ROC曲线发现RDW与其他观察指标比较,对于判断慢性心力衰竭具有重要临床意义。Logistic回归分析提示RDW与慢性心衰具有相关性(OR 3.83,95%CI 1.99~2.15,P〈0.01),RDW与hs-CRP具有相关性(OR 5.53,95%CI 3.28~4.51,P〈0.01)。结论 RDW与慢性心力衰竭的严重程度正相关,与贫血程度正相关,可以作为慢性心力衰竭合并贫血病情严重程度及预后观察指标。 Objective To study the levels of red blood cell distribution width (RDW) in chronic heart failure with anemia. Methods A total of 125 patients from our hospital were divided into two groups:chronic heart failure without and with anemia group.60 healthy individuals from health examination were used for the normal group. Red blood cell distribution width was detected using five-classification automatic blood analyzer. Results The levels of RDW in the chronic heart failure with anemia group had a significantly higher than other groups [(15.44+1.05)%〉 (14.46_+0.75)%〉 (12.38-+0.58)%, P〈0.01]; Bivariate Correlations analysis showed that RDW was positively correlated with NT-proBNP, uric acid (UA), high-sensitivity C-relative protein (hs-CRP) and serum creatinine (Cr) (r=0.78, P〈0.01; r=0.51, P〈0.01; r=0.42, P〈0.01; r=0.35, P〈0.01 ), but negatively correlated with hemoglobin (r=-0.72,P〈0.01). Multivariate logistic regression analysis showed that RDW was significantly associated with chronic heart failure and high-sensitivity C-rela- tive protein(hs-CRP)(OR 3.83,95%CI 1.99-2.15, P〈0.01 ), (OR 5.53,95% CI 3.28-4.51, P〈0.01). Conclusion RDW was positively correlated with worsening of chronic heart failure, negatively correlated with the levels of hemoglobin. RDW can be served as one of observing indexes of the worsening of chronic heart failure with anemia.
