普瑞巴林联合甲钴胺治疗带状疱疹疗效及对后遗神经痛不良反应的对比分析

Comparative analysis of efficacy and adverse reaction postherpetic neuralgia of pregabalin combined with mecobalamine in treatment of herpes zoster

目的 观察普瑞巴林联合甲钴胺治疗带状疱疹的疗效及对后遗神经痛不良反应的影响。方法 选择本院2014年2～11月皮肤科接收的带状疱疹后遗神经痛患者124例,随机分为普瑞巴林组与传统组各62例,基础治疗为吗啉胍片＋维生素C片,传统组在基础治疗上给予双氯芬酸钠肠溶缓释胶囊治疗,普瑞巴林组在基础治疗上给予普瑞巴林联合甲钴胺片治疗,观察两组患者2周后治疗效果,治疗后1周和2周疼痛面积、VAS评分、晚间睡眠时间、不良反应发生率及后遗神经痛发生率。结果 治疗2周后,普瑞巴林组总有效率明显高于传统组（χ2=11.273,P〈0.05）,两组患者疼痛面积、VAS评分、晚间睡眠时间等比较差异有统计学意义,随着时间变长变化明显（P〈0.05）,普瑞巴林组不良反应发生率明显低于传统组,差异具有统计学意义（χ2=10.127,P=0.001〈0.05）。治疗后1个月随访结果显示,普瑞巴林组后遗神经痛发生率明显低于传统组（χ2=9.021,P〈0.05）。结论 普瑞巴林联合甲钴胺具有提高带状疱疹治疗效果、快速缩小疼痛面积、减轻疼痛程度、延长晚间睡眠时间、降低不良反应发生率、降低后遗神经痛发生率等作用,值得推广。

Objective To compare the efficacy and adverse reaction postherpetic neuralgia of pregabalin combined with mecobalamine in the treatment of herpes zoster. Methods 124 patients with herpes zoster postherpetic neuralgia admitted to the dermatological department of our hospital from February to November 2014 were selected and randomly divided into the pregabalin group and the traditional group, with 62 patients in each group. The basic treatment was moroxydine tablets plus vitamin C tablets, on the basis of which the traditional group was given diclofenac sodium enteric-coated sustained release capsules while the pregabalin group was given pregabalin combined with mecobalamine tablets. The treatment effects after 2 weeks of treatment and the area of pain, VAS scores, night sleep time, incidences of adverse events and incidences of postherpetic neuralgia after 1 and 2 weeks treatment of the two groups were observed. Results After 2 weeks of treatment, the pregabalin group was significantly higher than the traditional group in the total effec- tive rate（x2=11.273, P〈0.05）, the two groups were different in the area of pain, VAS score and night sleep time, which presented a down trend with time （P〈0.05）, and the pregabalin group was significantly lower than the traditional group in the incidence of adverse reactions, with statistically significant differenees（χ2=10.127, P=0.001〈0.05）. The follow-up results 1 month after the treatment showed that the pregabalin group was significantly lower than the traditional group in the incidence of postherpetic neuralgia （χ2=9.021, P〈0.05）. Conclusion Pregabalin combined with mecobalamine has treatment effects on improving herpes zoster and can rapidly shrink area of pain, alleviate degree of pain, prolong night sleep time, reduce the incidence of adverse reactions and reduce the incidence of postherpetic neuralgia, thereby worthy of promotion.
