来氟米特、环磷酰胺联合糖皮质激素对系统性红斑狼疮患者肾功能与免疫功能的影响

Effects of Leflunomide,Cyclophosphamide Combined Glucocorticoid on Renal Function and Immune Function in Patients with Systemic Lupus Erythematosus(SLE)

目的:探讨来氟米特、环磷酰胺联合糖皮质激素对系统性红斑狼疮患者肾功能与免疫功能的影响。方法:选取我院内分泌风湿科住院部系统性红斑狼疮患者79例,采用随机数字表法将患者分为研究组与对照组,对照组39例,采用环磷酰胺联合糖皮质激素治疗,观察组40例,采用来氟米特联合糖皮质激素治疗,采用电化学发光法检测肾功能指标(血清肌酐Scr、尿素氮BUN)与免疫功能指标(Ig A、Ig G、Ig M、补体C3与补体C4)水平,同时测定24h尿蛋白水平。结果:研究组患者治疗总有效率明显高于对照组(P<0.05),两组系统性红斑狼疮疾病活动指数(SLEDAI)评分、肾功能指标(Scr、BUN、24h尿蛋白)与免疫功能指标水平比较差异无统计学意义(P>0.05)。结论:来氟米特、环磷酰胺联合糖皮质激素治疗系统性红斑狼疮患者的临床疗效显著,有助于显著改善肾功能与免疫功能,且药物安全性较高。

Objective: To investigate the effect of the combination of glucocorticoid and cyclophosphamide on renal function and immune function in patients with SLE.Methods:79 cases of patients with SLE were selected from endocrine rheumatology department of our hospital,using the random number table method,all were divided into study group and control group,control group 39 cases,were given cyclophosphamide combined glucocorticoid therapy,observation group,40 cases,were given leflunomide combined glucocorticoid therapy.Results:The total effective rate ofstudy group were significantly higher than control group,the drug adverse reactions incidence were significantly lower than control group,two groups compared with a statistically significant difference(P<0.05).Before the treatment,SLEDAI score,renal function index(Scr,BUN,24h urine protein)and immune function index between the two groups were compared with no statistically significant difference(P>0.05).Conclusion:The clinical curative effect of leflunomide,cyclophosphamide combined glucocorticoid forpatients with SLE is distinct,it helps improving renal function and immune function,and drug safety is higher.
