中药问题处方的审核分析与对策

Audit Analysis and Countermeasure of the Prescription of Chinese Medicine

目的研究并分析中药问题处方的审核过程、存在的问题及改善措施.方法：回顾性分析我院2015年1月～2015年12月1 6 4 9张中药处方,对其中出现的问题进行分析.结果：在8 9 0张（占5 4 . 0 %）中成药处方中,有2 7张出现了调剂差错,发生率为3 . 0 %;7 5 9张（占4 6 . 0 %）中草药处方中,有1 5张出现了调剂差错,发生率为2 . 0 %;在总1 6 4 9张中,有3 7张出现毒性药物超量的问题,发生率为2 . 2 %,这些易出现超量使用的毒性药品为仙茅、半夏、吴茱萸、蛇床子、白附子、全虫、天南星等.结论：在开具中药处方时,应通过严格的审核制度及时发现处方中可能存在的问题,减少问题处方的出现,为患者提供用药安全保障.

Objective: To study and analyze the audit process,existing problems and improvement measures of Chinese medicine prescription.Methods:Retrospective analysis of our hospital in Jan 2015~2015 Dec 1 649 prescriptions of Chinese medicine,the problem of which was analyzed.Results:In the 890 prescriptions of Chinese medicine,there are 27 out of dispensing errors,the incidence was 3%; 759 prescriptions of Chinese herbal medicine,there are 15 out of dispensing errors,the incidence was 2%.In total 1 649 pieces of TCM prescriptions,37 sheet drug toxicity of excessive problem and the incidence rate was 2.2%,and prone to excessive use of toxic drugs for Curculigo rhizome,Rhizoma Pinelliae,Evodia rutaecarpa,common cnidium fruit,Rhizoma typhon,scorpion,Tian Naxing.Conclusion:In the prescription of TCM,it should be through strict examination and approval system in time to find the problems in the prescription,reduce the emergence of the problem,to provide patients with medication safety.
