血必净注射液联合乌司他丁对重症急性胰腺炎患者血清内毒素及炎症因子的影响

Effects of Xuebijing injection combined with ulinastatin on endotoxin and inflammatory factors in patients with severe acute pancreatitis

目的:探讨血必净注射液联合乌司他丁对重症急性胰腺炎患者血清内毒素及炎症因子的影响。方法:选取在我院重症医学科治疗的重症急性胰腺炎患者52例,随机分为A组和B组,每组各26例。两组均给予常规治疗及必要的营养支持,A组在此基础上加用乌司他丁,B组在A组治疗的基础上加用血必净注射液。治疗前、治疗后3d及7d,检测血清内毒素、hs-CRP、IL-6、IL-8、IL-10和TNF-α的水平。结果:两组治疗后hs-CRP和内毒素水平均较治疗前降低,且B组较A组下降更显著,差异均具有统计学意义(P<0.05);两组IL-6、IL-8及TNF-α水平均较治疗前降低,且B组下降幅度大于A组,差异均具有统计学意义(P<0.05);而IL-10水平较治疗前升高,且B组升高幅度大于A组,差异均具有统计学意义(P<0.05)。结论:血必净注射液联合乌司他丁可降低重症急性胰腺炎患者内毒素水平和调控炎症因子的释放。

Objective:To investigate the effects of Xuebijing injection combined with ulinastatin on endotoxin and inflammatory factors in patients with severe acute pancreatitis.Methods:A total of 52 patients with severe acute pancreatitis treated in our hospital were selected and divided into two groups:group A(n=26)and group B(n=26).All the patients were given conventional treatment and necessary nutrition support.Patients of group A was treated with ulinastatin on the basis of conventional treatment and nutrition support,patients of group B was treated with Xuebijing injection on the basis of group A.Before and after treatment,the serum endotoxin,hs-CRP,IL-6,IL-8,IL-10 and TNF-αwere detected anc compared between the two groups.Results:After treatment,the hs-CRP and endotoxin in the two groups were significant lower than that before treatment(P<0.05),and the decrease in group B was more significant than that in group A(P<0.05).The levels of IL-6,IL-8and TNF-αof the two groups were significantly lower than before treatment(P<0.05),and the decrease in group B was more obvious than that of group A(P< 0.05).However,levels of IL-10 of the two groups were significantly increased than that before treatment(P<0.05),and the increase in group B was more obvious than that of group A(P<0.05).Conclusion:Xuebijing injection combined with ulinastatin can significantly improve the clinical treatment effects through reducing serum endotoxin levels and inhibiting the release of inflammatory factors of patients with severe acute pancreatitis.
