自拟甲参汤治疗代偿期肝硬化肝脾血瘀证的临床研究

The clinical observation of Jia Shen Tang in the treatment of compensatory cirrhosis

目的探讨自拟甲参汤治疗代偿期肝硬化(肝脾血瘀证)的临床效果,客观评价甲参汤治疗代偿期肝硬化的可行性。方法选取90例代偿期肝硬化患者,随机分为治疗组(n=45)和对照组(n=45)。对照组釆用常规西医对症治疗。治疗组在充分告知情况下,在对照组基础上加用自拟甲参汤(鳖甲+丹参)治疗。治疗30天后,观察中医证候、肝功能、肝纤维化指标、门静脉主干内径、脾脏体积等指标,数据进行统计分析。结果治疗组总有效率为95.6%,对照组为75.6%,两组比较差异有统计学意义(P<0.05)。中医证候改善、肝功能检测、肝纤维化指标检测、门静脉压力、脾脏体积等,组间比较差异有统计学意义(P<0.05)。结论自拟甲参汤治疗代偿期肝硬化疗效确切,可以缓解临床症状、恢复肝功能、减轻肝纤维化、降低门静脉压力的作用,且具有临床安全性。

Objective： Through the clinical observation of Jia Shen Tang in the treatment of componsatoD＇ cirrhosis （liver and spleen stasis syndrome ）, obtain an objective evaluation about the clinical efficacy and security. Methods： 60 patients with liver cirrhosis were randomly divided into treatment group （ n = 45 ） and control group （ n = 45 ）. The control group was treated with routine western medicine and Western medicine. In the treatment group were fully informed of cases, on the ba- sis of the control group treated with serf-mede Decoction in the treatment of a ginseng. After 30 days of treatment, TCM symptoms, liver function, liver fibrosis index, the inner diameter of portal vein, splenic volume and other indicators were observed, and the data were statistically analyzed. Results： The total effective rate was 95.6% in treatment group and 75.6% in control group, The difference between the two groups was statistically significant （P 〈0. 05）. TCM symptoms improved, liver function test, liver fibrosis index test, portal vein pressure, spleen volume, etc. The difference was statistically significant （P 〈 0. 05 ）. Conclusion： The curative effect of serf-made a ginseng decoction in the treatment of decompensated liver cirrhosis is exact, and can alleviate the clinical symptoms, recovery of liver function, alleviate hepatic fibrosis, decrease portal pressure and with clinical safety.
