新泰市脑梗死二级预防中阿司匹林应用现况及复发情况调查

The cross-sectional study on use of aspirin in secondary prevention and recurrence of patients with cerebral infarction in Xintai

目的调查新泰市脑梗死患者二级预防中阿司匹林应用现况,探讨阿司匹林应用不规范的原因及阿司匹林应用对脑梗死复发的影响。方法通过电话随访、门诊随访方式对2013年6月—2015年6月在我院神经内一科就诊的529例脑梗死患者阿司匹林服用情况进行现况调查,分析干扰阿司匹林服用的主要影响因素及阿司匹林对脑梗死复发的影响。结果 （1）本地区脑梗死患者阿司匹林应用率较低。（2）不同教育程度、收入水平、高血压病史、糖尿病病史、用药指导的脑梗死患者阿司匹林应用情况进行比较,其差异具有统计学意义。（3）规范服用阿司匹林组脑梗死患者复发率明显低于未规范服用组（χ^2=24.317,P〈0.001）。结论本地区脑梗死患者阿司匹林应用形势仍较为严峻,影响阿司匹林应用的因素较多,规律服用阿司匹林组脑梗死患者复发率明显低于不规律服用组,我们应加强二级预防知识学习,加强对脑梗死患者健康教育,提高阿司匹林治疗的知晓率及重要性。

Objective： Objective To investigate the application status of aspirin in the secondary prevention among pa- tients with cerebral infarction in the city of Xintai, and explore the causes for inappropriate use of aspirin and the effect of aspirin on the recurrence of cerebral infarction. Methods ： 529 patients were followed up about the use of aspirin by tele- phone and clinic service from June 2013 to June 2015. The main causes for inappropriate use of aspirin and the effect of as- pirin on the recurrence of cerebral infarction were explored. Results： （ 1 ） The application rate of aspirin among patients with cerebral infarction was very low. （2） There were significant differences in the use of aspirin between patients with high education, high income, diabetes mellitus, hypertension, normative health education and those without. （3）The recur- rence of cerebral infarction in patients in normatiye aspirin use group was significantly lower than that of patients in non- normative group （X2 = 24. 317,P 〈 0. 001 ）. Conclusion： The use rate in the secondary prevention among patients with cere- bral infarction is not optimistic. Clinicians need to learn the secondary prevention knowledge hardly and improve their un- derstanding of cerebral infarction. They also need to strengthen the health education in order to improve the cerebral infarc- tion aware and the importance of aspirin use.
